AMENDMENT TO LEASE WITH CENTRAL ARIZONA SHELTER SERVICES, LLC Retroactively approve and execute Amendment No. 1 to lease No. MC10166 with Central Arizona Shelter Services, LLC, Lessee, for 7,687 square feet of warehouse space located at 1214 W. Madison, Phoenix , AZ. The term of the lease commences September 18, 2008, and terminates September 17, 2009, with an option to renew for one additional year. The annual rental rate is $1.00 per year. The Lessee wishes to utilize this facility as a “Low Demand” shelter. This lease contains a 90-day termination provision and a six-month holdover. The Facility Review Committee endorsed this action via electronic voting during August 2008. (C-18-08-010-4-01)

49. AMENDMENT TO LEASE WITH CENTRAL ARIZONA SHELTER SERVICES, INC Approve and authorize the Chairman to execute, Amendment No. 3 to NNN lease No. MC10166 with Central Arizona Shelter Services, Inc., Lessor as successor in interest to Central Arizona Shelter Services LLC, and Lessee, for 7,687 square feet of warehouse space located at 1214 W. Madison Street, Phoenix, AZ. The term of the lease is amended to add ten renewal terms which shall each be one year in length and shall automatically renew at the end of each renewal term unless terminated be either Lessee or Lessor. The lease term commences September 18, 2010 and terminates September 17, 2020. The annual rental rate is $1.00 per year. The Lessee wishes to utilize this facility as a “Low Demand” shelter. This lease contains a 90-day termination provision and a 6-month holdover. This amendment was endorsed by the Facility Review Committee on May 27, 2010. (C-18-08-010-4-03)
