30. AMENDMENT TO MARICOPA COUNTY EMPLOYEES COMBINED CHARITABLE CAMPAIGN AND VALLEY OF THE SUN UNITED WAY CAMPAIGN AND FISCAL SPONSOR AGREEMENT Approve Amendment No. 2 (C-20-18-005-3-03) to the Agreement between Valley of the Sun United Way (VSUW) and Maricopa County through the County Administration. The purpose of this amendment is to extend Agreement and Amend VSUW Employee Assistance Fund Guidelines (Exhibit A). This amendment is effective from 06/01/2020 to 05/31/2021. All other terms and conditions of the Agreement remain in full force and effect. The sections in the amended Guidelines related to the COVID-19/Coronavirus pandemic are retroactive to April 1, 2020. As the fiscal sponsor, VSUW will provide services for the annual Combined Charitable Campaign and the Employees Assisting and Supporting Employees (E.A.S.E.) fund. The VSUW Employee Assistance Fund Guidelines (Exhibit A) have been amended to open up E.A.S.E. to financial hardship that results from the COVID-19/Coronavirus pandemic. (C-20-18-005-3-03)

28. AMENDMENT TO CAMPAIGN AND FISCAL SPONSOR AGREEMENT WITH VALLEY OF THE SUN UNITED WAY Approve Amendment No. 5 to the Campaign Services and Fiscal Sponsor Agreement between Valley of the Sun United Way (VSUW) and Maricopa County through the County Administration. As the fiscal sponsor, VSUW provides services for the annual Combined Charitable Campaign and the Employee s Assisting and Supporting Employees (E.A.S.E.) fund. The VSUW Employee Assistance Fund Guidelines (Exhibit A) have been amended to reflect procedural changes implemented as a result of COVID-19, namely that checks will be mailed directly to vendors listed on approved application and that the County shall direct if checks should be sent more expeditiously than regular mail (i.e., via overnight courier). This authorization provides a one-year extension of the Agreement, retroactive from June 1, 2022 through May 31, 2023. All other terms and conditions of the Agreement remain in full force and effect.
