28. ADMINISTRATIVE CHANGE TO RECOGNIZE NEW OWNER/LESSOR UNDER LEASE L-7330 Approve an administrative change to Lease Agreement No. C-22-03-110-4/L-7330 to recognize 2150 Country Club, LLC, an Arizona limited liability company, as the new property owner and successor Lessor and Cushman & Wakefield US, Inc. as the new property manager with regards to the Human Services Department office space lease at 2150 S. Country Club Road, Mesa, AZ. The County recently received formal notice on March 21, 2019 that 2150 Country Club, LLC is the new property owner and successors in interest to BAR/JCR PHX Flex Investors, LLC via Special Warranty Deed as recorded with the County Recorder on March 20, 2019, at instrument number 2019-0192867. All rent payments and Notices due under the Agreement shall be sent to 2150 Country Club, LLC c/o Cushman & Wakefield, 7420 S. Kyrene Road, Suite 101, Tempe, AZ 85283. The lease expires June 30, 2021. Section 1 of Amendment 3 of the lease allows County to terminate upon 120-day written notice to the Lessor. This lease is funded through a grant by the U.S. Department of Health and Human (DHHS)/Administration for Children and Families (ACF), Office of Head Start (OHS) (C-22-16-011-G & C-22-15-036-G). This agreement does not impact the County General Funds. Supervisory District: 2 (C-22-03-110-4-10)
