33. LICENSE AGREEMENT WITH CHILD AND FAMILY RESOURCES FOR CO-LOCATION IN THE MARICOPA WORKFORCE CONNECTIONS ONE STOP CAREER CENTER Approve License Agreement (P-50131) with Child and Family Resources (Licensee) a non-profit organization, for the purpose of co-locating Licensee in the Maricopa Workforce Connections (MWC), West Valley One Stop Career Center. This Agreement allows Licensee’s staff to maintain direct service space within the center and provides for funding arrangements whereas Licensee will pay a monthly fee of $34.36 for costs associated with the operation of the center. This License Agreement is a resource sharing agreement. This License Agreement is effective upon approval and signature of both parties and will expire on June 30, 2011. Indirect funds do not apply to this agreement since all funds collected under this agreement will be applied to the costs associated with the operation of the center. There are no County General funds associated with this License Agreement. The center is located at 1840 N. 95th Avenue, Phoenix, AZ. (C-22-10-060-3-00)

56. AMENDMENT TO THE LICENSE AGREEMENT WITH CHILD AND FAMILY RESOURCES FOR CO-LOCATION IN THE MARICOPA WORKFORCE CONNECTIONS ONE STOP CAREER CENTER Approve Amendment No. 1 to the License Agreement (P-50131) between Child and Family Resources, a non-profit organization and Maricopa County administered by its Human Services Department, Maricopa Workforce Connections (MWC). The Amendment addresses the following items: • Maricopa County exercises the option to renew the term of the Agreement for an additional year. The Agreement shall expire June 30, 2012. • Removes Exhibit “A” Fee Schedule from the Agreement. • Attach Exhibits 1 - Guidelines for all partner agencies that are c-located in the Maricopa Workforce Connections One Stop Career Centers; Exhibit 2 • Attach Exhibit 2 - West Valley One Stop Career Center Operating Costs • Attach Exhibit 3 - Fee Schedule Through this Amendment, Child and Family Resources will continue to maintain direct services space within the MWC West Valley One Stop Career Center. The West Valley One Stop Career Center is located at 1840 N. 95th Ave., Phoenix, AZ. Child and Family resource will pay on a monthly basis for co-location costs associated with the operation of the One Stop Career Centers. Indirect funds do not apply to this Amendment since all funds collected under this Agreement will be applied to the costs associated with the operation of the One Stop Career Center.
