31. AGREEMENT WITH CATHOLIC HEALTHCARE WEST DBA CHANDLER REGIONAL HOSPITAL FOR ORAL HEALTH SERVICES TO HEAD START CHILDREN AND FAMILIES Approve a non-financial Agreement between Maricopa County, through its Human Services Department, Education Division, and Catholic Healthcare West dba Chandler Regional Hospital(CHW) to establish the terms and conditions for CHW to provide oral health services to participating Head Start children and their families in the East Valley. The term of this agreement is for five years commencing with the Chairman of the Board’s signature and ending on January 1, 2016. Staff from CHW will provide oral health screenings, oral health education, and fluoride varnishes to Head Start children and families at classroom sites at no cost to the County or the recipients. Upon completion of these screenings, CHW will provide referrals for follow-up care as required. (C-22-11-057-3-00)
