32. LICENSE AND FEE AGREEMENT WITH MAXIMUS HUMAN SERVICES FOR USE OF THE MARICOPA WORKFORCE CONNECTIONS ONE STOP CAREER CENTERS Approve License Agreement P-50178 with MAXIMUS Human Services (Licensee), to allow Licensee the use of the Maricopa Workforce Connections (MWC), One Stop Career Centers Monday through Friday. Licensee will use the Center to provide case management services to Licensee participants. Licensee shall pay $1,005.53 monthly for costs associated with the operation of the One Stop Career Centers. The License Agreement is retroactive to July 1, 2011 and will expire June 30, 2013, with the option to renew the license two times for a period of one year each. Indirect funds do not apply to this agreement since all funds collected under this agreement will be applied to the costs associated to the operation of the One Stop Career Centers. This Agreement does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. 42-17105. The overall grant budget will be adjusted as necessary to accommodate this Agreement through a future reconciliation. There are no County General funds associated with this License Agreement. The West Valley One Stop Career Center is located at 1840 N. 95th Avenue, Phoenix, AZ and the East Valley One Stop Career Center is located at 735 N. 48. AMENDMENT TO LICENSE AGREEMENT WITH MAXIMUS HUMAN SERVICES, INC. FOR USE OF THE MARICOPA WORKFORCE CONNECTIONS ONE STOP CAREER CENTERS Approve, and authorize the Chairman to execute, License Amendment P-50178 with Maximus Human Services, Inc. to allow Maximus to utilize office space and equipment at the Maricopa Workforce Connections (MWC) One-Stop Career Centers. Maximus will provide In-kind contribution in place of cost reimbursement for the use of the space in the One-Stop Career Centers. The total monthly In-kind contribution value shall be $5,057 as reimbursement toward the proportionate share of costs associated with the operation of the One-Stop Career Centers. Maximus shall provide monthly reports indicating the total value on In-kind contribution. The County shall evaluate Center operations costs on a yearly basis and reimbursement costs may be adjusted for increases in operational costs. The terms of the amendment shall be effective upon signature by both parties. The License Agreement shall expire on June 30, 2014. The agreement may be renewed for two additional one year terms upon agreement by both parties. Agencies co-located in the One-Stop Career Centers must reimburse the County for their proportionate share of costs for the operation of the Career Centers. All funds collected under this agreement will be applied against the costs associated to the operation of the One-Stop Career Centers. There are no indirect costs collected on this agreement. 73. AMENDMENT TO LICENSE AGREEMENT WITH MAXIMUS HUMAN SERVICES, INC. Approve, and authorize the Chairman to execute, License Amendment #2 P-50178 with Maximus Human Services, Inc. to allow Maximus to continue to utilize office space and equipment at the Maricopa Workforce Connections (MWC) One-Stop Career Centers. The amendment extends the agreement term to June 30, 2015 and adjusts the monthly reimbursement amount to be provided by Maximus. The terms of the amendment shall be effective upon signature by both parties. Maximus will provide In-kind contribution in place of cost reimbursement for the use of the space in the One-Stop Career Centers. The total monthly In-kind contribution value shall be $5,104 as reimbursement toward the proportionate share of costs associated with the operation of the One-Stop Career Centers. Maximus shall provide monthly reports indicating the total value of In-kind contribution. There are no indirect costs collected on this agreement. Any necessary grant adjustments to accommodate this item will be made through a future reconciliation. This agreement does not impact the County General funds. Agencies co-located in the One-Stop Career Centers must reimburse the County for their proportionate share of costs for the operation of the Career Centers. Maximus shall provide In-Kind monthly contribution by assigning staff to work to provide front desk customer service, assistance with workshop facilitation (i.e.: Job Club and assistance in resource room as needed).
