41. IGA WITH THE CITY OF TOLLESON FOR CRISIS CASE MANAGEMENT AND FINANCIAL ASSISTANCE SERVICES Approve a non-financial, Intergovernmental Agreement (IGA) between the City of Tolleson and Maricopa County (administered by its Human Services Department – “MCHSD”) for the provision of Crisis Case Management and Financial Assistance Services . MCHSD has had an ongoing successful IGA in place with the City of Tolleson, and requests approval for the term of this agreement; from July 1, 2013, through June 30, 2014. The City of Tolleson is providing all administrative funding required for the operation of the City of Tolleson Community Action Program (CAP) without financial support from Maricopa County. MCHSD will provide the City access to HSD Online, MCHSD’s client database and portal to directly submit client applications. MCHSD will then approve final expenditure of appropriate financial assistance funds to be utilized on behalf of eligible clients. (C-22-13-061-3-00)

50. AMENDMENT TO THE IGA WITH THE CITY OF TOLLESON Approve amendment #1 to the non-financial Intergovernmental Agreement (IGA) between the City of Tolleson and Maricopa County, administered by its Human Services Department (MCHSD). The County will exercise its option to renew the agreement for a one year renewal, based on Contractor performance. The amendment will extend the agreement term to June 30, 2015 for the provision of Crisis Case Management and Financial Assistance Services for the City of Tolleson Community Service Area. The amendment will be effective July 1, 2014.The City of Tolleson will provide all administrative funding required for the operation of the City of Tolleson Community Action Program (CAP) without financial support from Maricopa County.
