42. AMENDMENT TO SUBRECIPIENT AGREEMENT WITH TRELLIS FOR COMMUNITY DEVELOPMENT BLOCK GRANT PROGRAM Approve non-financial Amendment No. 1 to the Agreement (C-22-14-009-3) between Maricopa County administered through its Human Services Department and Neighborhood Services of Phoenix (“Subrecipient”) (collectively, “Parties”). The Parties entered into a Subrecipient Agreement with a term of July 1, 2013 through August 21, 2015. On March 2, 2015, Neighborhood Housing Services of Phoenix, Inc. filed an amendment to its Articles of Incorporation with the Arizona Corporation Commission changing its name to TRELLIS. The date of the adoption for the name change amendment was February 26, 2015. The purpose of the amendment is to acknowledge the Subrecipient’s name change, and to extend the expiration term of the agreement from August 21, 2015 to June 30, 2016 retroactive to and effective on August 22, 2015. The Amendment will also revise the Implementation Schedule to agree with the extended expiration date as more specifically set forth in the attached Amendment. Project activities under this Agreement include counseling services for homebuyer education and financial literacy workshops for low-to-moderate income clients. This Agreement is funded by a grant from the U. S. Department of Housing and Urban Development (HUD), (C-22-13-052-G-01). (C-22-14-009-3-01)
