31. AFFILIATION AGREEMENT WITH PRIORITY NUTRITION CARE LLC, HINGHAM, MA Approve an Affiliation Agreement with PRIORITY NUTRITION CARE LLC (“PNC”), located at 14 Holly Street., Hingham, MA 02043, to provide a learning experience for PNC interns enrolled in the Priority Nutrition Care Distance Dietetic Internship program. This agreement is non-financial, and the term shall commence January 13, 2014 and extend until terminated by either party upon at least thirty (30) days prior written notice. The purpose of this Affiliation Agreement is to establish a cooperative relationship between PNC and the Maricopa County Human Services Department, Education Division (Head Start Program), and to outline the responsibilities of both parties as they contribute to the learning experience of Interns in the PNC Dietetic Internship Program.
