39. PARTNERSHIP AGREEMENT WITH COMMUNITY INFORMATION AND REFERRAL SERVICES Approve a financial Partnership Agreement and Addendum between Community Information and Referral Services and Maricopa County administered by its Human Services Department (HSD). The purpose of the agreement is to allow HSD access to the Maricopa Homeless Management Information System (Maricopa HMIS), through User Licenses. Through this agreement, Human Services department staff shall have access to input, use and receive information concerning homeless clients. Maricopa County HSD shall request 2 User Licenses at a cost of $150 per license per year. The agreement is for a five (5) year term. This agreement shall not impact the County’s General Fund. (C-22-15-044-3-00) 40. LEASE AGREEMENT WITH SON RISE FAITH COMMUNITY CENTER, INC. Approve, and authorize the Chairman to execute, Lease Agreement L-7457 with Son Rise Faith Community Center, Inc., an Arizona non-profit corporation, as Lessor, for exclusive use of classroom and restroom space and non-exclusive use of playground area at 800 West Galveston Street, Chandler, Arizona. The Agreement will allow the County use of the site for one Head Start classroom and one Early Head Start classroom and use of a playground area. The Agreement term is for approximately five (5) years and will commence upon execution of both Parties and expire May 30, 2020. The initial rate upon execution will be One Hundred Dollars ($100.00) during which time tenant improvements will be made to the site. Once occupied, the rate will adjust to Nine Hundred Dollars ($900.00) per month. Maricopa County shall pay for utilities costs, estimated at One Hundred Dollars ($100.00) per month. Maricopa County is responsible for providing janitorial services. Monthly rental rate and utilities costs will be revisited at the end of the second year term, May 30, 2017. By mutual agreement the term may be extended for one three (3) year term. Either Party may terminate the Agreement with a 30-day notice to the other party. This Agreement is funded through the grant provided by U.S. Department of Health and Human Services Head Start Program (C-22-14-058-G-00). This agreement does not impact the County General Funds.
