45. AMENDMENT TO SOLE SOURCE CONTRACT WITH LODESTAR DAY RESOURCE CENTER Approve a financial Amendment No. 3 to the Sole Source Contract with Lodestar Day Resource Center (LDRC), a non-profit community based organization. The purpose of the contract amendment is to increase funding by $225,000 from $540,789 to $765,789. The amendment also replaces Attachment B Monthly Payment Request. The increased funding availability period shall be July 1, 2015 to June 30, 2016. All other terms and conditions of the original agreement and previous amendments remain in full force and effect. Funding for this amendment will be reimbursed through the Human Services Department appropriated County General Funds, Community Development Block Grant funded through HUD and the IGA with Arizona Department of Economic Security for the provision for Community Action Program services. A-4. AMENDMENT TO SOLE SOURCE CONTRACT WITH LODESTAR DAY RESOURCE CENTER Approve a financial Amendment No. 4 to the Sole Source Contract with Lodestar Day Resource Center (LDRC), a non-profit community based organization. The purpose of the Amendment is to address the following items: • Pursuant to Paragraph 4 Agreement Renewal: extend the Agreement term from June 30, 2016 to June 30, 2017. Approval and acceptance of Amendment No. 4 will extend the expiration date of the Agreement. • Amendment No. 4 shall be effective on July 1, 2016 • The County shall provide LDRC with $376,475 for the period of July 1, 2016 to October 31, 2016. • Unexpended funds from the Original Agreement and prior Amendments will not carryover into the agreement extension period. • Incorporate by reference the Payment Claim Form in Attachment B as part of this Agreement. • This Agreement is subject to and incorporates by reference the statutory language of A.R.S. §38-511. • The foregoing paragraphs contain all the changes to the Agreement. All other terms of the original Agreement and Amendments not amended hereby, remain in full force and effect as approved and amended. As provided by U.S. Department of Housing and Urban Development (HUD): i. 14.218, Community Development Block Grant $85,000 Program Year 2015 ii. 14.218, Community Development Block Grant $76,475 Program Year 2016 b. 93-569 Community Services Block Grant (CSBG) $150,588; II. Incorporate by reference Attachment C – Program Requirements into the Agreement as required for receipt of federal funds; III. Parties agree that the statutory language of A.R.S. §38-511 are applicable to this Amendment and incorporate the same by reference; IV. AMENDMENT TO SOLE SOURCE CONTRACT WITH LODESTAR DAY RESOURCE CENTER Approve Amendment No. 6 to the Sole Source Contract with Lodestar Day Resource Center (LDRC), a non-profit community based organization. The purpose of the Amendment is to address the following items: I. Revised Section 8.0 Responsibilities of Organization; II. Reduce funding amount from $376,476 to $278,382 for the period of July 1, 2016 to October 31, 2016 III. Substitute the revised Payment Claim Form in Attachment B into the Agreement IV. The Amendment shall be effective upon approval and signature by both parties. V. The foregoing paragraphs contain all the changes to the Agreement. All other terms of the original Agreement and Amendments not amended hereby, remain in full force and effect as approved and amended. VI. This amendment is subject to termination pursuant to the provisions of A.R.S. § 38-511. LDRC provides services to homeless individuals in the Human Services Campus located at 1125 W. The purpose of the Amendment is to address the following items: I. Pursuant to paragraph 4 of the Agreement, amend Paragraph 3.1 extending the expiration date from October 31, 2016 to February 28, 2017, effective October 31, 2016. II. Amend Paragraph 5.0 a (ii) which identifies funding sources as defined by Catalog of Federal Domestic Assistance (CFDA) and revise funding source 14.218, Community Development Block Grant (CDBG) from $76,475 to $115,000 for Program Year 2016. III. Notwithstanding any prior Amendment to the contrary, amend the Agreement to permit unexpended authorized funding for the period of July 1, 2016 until October 31, 2016 to be to be carried forward into the extended contract period from October 31, 2016 to February 28, 2017. IV. Parties agree that the statutory language of A.R.S. §38-511 are applicable to this Amendment and incorporate the same by reference. V. The foregoing paragraphs contain all the changes to the Agreement as amended. All other terms of the original Agreement and Amendments not amended hereby, remain in full force and effect as approved and amended.
