35. DATA SHARING AGREEMENT WITH ARIZONA DEPARTMENT OF HEALTH SERVICES OFFICE OF ENVIRONMENTAL HEALTH Approve a non-financial Data Sharing Agreement between Arizona Department of Health Services Office of Environmental Health (ADHS) and Maricopa County administered by its the Human Services Department for the Head Start program. The agreement will allow for the sharing of information regarding blood lead screening and testing data. The agreement will also allow for working collaboratively to implement actions to increase blood lead screening and testing in enrolled participants in the Early Head Start and Head Start program, as well as to coordinate follow up testing and family education as needed. The Agreement shall commence upon signature by both parties and will terminate five (5) years after the date of full execution. The Agreement may be terminated by either party upon sixty (60) calendar day’s written notice to the other Party. (C-22-15-066-3-00)
