44. CONTRACT WITH BANNER HEALTH-OLIVE BRANCH SENIOR CENTER Approve a contract between Banner Health, an Arizona non-profit d/b/a Banner Home Care non-profit, community based organization, referred to as “Olive Branch Senior Center” and Maricopa County, administered by its Human Services Department, in the not-to-exceed amount of $10,000. The purpose of the contract is to provide Olive Branch Senior Center with funding for the provision of support services to elderly residents in the Sun City community. The term of this contract is retroactive to July 1, 2015 through June 30, 2016. Funding for this contract is from the General Fund. The Olive Branch Senior Center is the only entity that provides a combination of senior services and community action program (CAP) services in Sun City. Olive Branch Senior Center meets the needs of low-income elderly residents in the Sun City area by providing congregate meals, food bags, socialization, and referrals. Additionally, they provide CAP utility assistance on behalf of Maricopa County, which assists residents with high utility bills. (C-22-15-070-3-00)

49. AMENDMENT TO SERVICE AGREEMENT WITH BANNER HEALTH DBA BANNER HOME CARE FOR OLIVE BRANCH SENIOR CENTER Approve Amendment No. 1 between Banner Health, an Arizona non-profit d/b/a Banner Home Care non-profit, community based organization and Maricopa County, administered by its Human Services Department. The purpose of the Amendment to address the following: • Correct the name of the contracting Party to Banner Health d/b/a Banner Home Care, this change will remove the reference of Olive Branch Senior Center as the contracting Party. • Pursuant to Paragraph 4 Agreement Renewal extend the Agreement term from June 30, 2016 to June 30, 2017. Approval and acceptance of Amendment No. 1 will extend the expiration date of the Agreement. • Amendment No. 1 shall be effective on July 1, 2016. • The County shall provide Banner with $10,000 for the period of July 1, 2016 to June 30, 2017. • Funding shall be provided in four equal increments of $2,500 on a Quarterly basis within ten (10) business days after the end of each quarter of the fiscal year: July 1, 2016 to September 30, 2016; October 1, 2016 to December 31, 2016; January 1, 2017 to March 30, 2017: April 1, 2017 to June 30, 2017. • This agreement is subject to and incorporates by reference the statutory language of A.R.S. §38-511. • The foregoing paragraphs contain all the changes to the Agreement. All other terms and conditions of the original Agreement as amended remain the same and are in full force and effect as approved and amended. Funding for this contract is from the General Fund.
