The purpose of the Amendment is for the County to provide City of Avondale with 2017 HOME Investment Partnerships Program funds in the amount not-to exceed $134,240 for 2 new Scope of Work project activities, which includes the: 1) Homebuyer assistance program and increase the number of households assisted by 2, funding amount for activity shall not exceed $70,000; and 2) A single family housing rehabilitation for housing rehabilitation and administration costs and increase the number of households assisted by 1, funding amount for activity shall not exceed $64,240. This amendment increases the total funded amount to be $268,338. The Amendment shall also extend the Agreement term from March 9, 2019 to September 30, 2021. Extending the contract termination date will provide the City with additional time to complete the new scopes of work associated with the 2017 HOME funds. The purpose of the Agreement is for Catholic Charities Community Services Inc. to acquire and rehabilitate a single family home to be used as emergency housing for homeless youth aged 13-19. The County will provide the Subrecipient with Community Development Block Grant (CDBG) and Neighborhood Stabilization Program (NSP) funds in the amount not-to exceed $330,000. The term of the Agreement is July 1, 2018 and will expire June 30, 2019. SUBRECIPIENT AGREEMENT WITH REBUILDING TOGETHER VALLEY OF THE SUN FOR COMMUNITY DEVELOPMENT BLOCK GRANT ACTIVITIES Approve a Subrecipient Agreement between the Rebuilding Together, Valley of the Sun (referred to as the “Subrecipient”) and Maricopa County (the “County”) administered through its Human Services Department. The Subrecipient and County are collectively referred to herein as the “Parties” and individually as a “Party.” The purpose of the Agreement is for the Subrecipient to provide home assessments to identify health and safety concerns to owner occupied homes and install wheelchair ramps; ADA compliant door handles; create low-rise steps; install bathroom safety devices (tub rails, tub transfer benches, toilet riser seats, hand-held showers, shower seats, and/or install grab bars and handrails and other safety items. The Subrecipient shall provide services to 50 homes in the Town of Guadalupe. The County shall provide Subrecipient with Community Development Block Grant (CDBG) funds in the amount not-to exceed $25,000. The term of the Agreement shall begin on September 19, 2018 through June 30, 2020. Also request approval for the Human Services Department Director to approve and execute changes as listed below: Pursuant to Section II Special Provisions paragraph K. ADDITIONAL CERTIFICATIONS AND WARRANTIES Subparagraph 1. “r” Changes – The County may, at any time, by written Change Order/Administrative Change executed by the Director of the Human Services Department, make changes within the general scope of this Agreement in any one or more of the following areas: i. Modifications to the project implementation schedule as long as it does not extend or shorten the term of the Agreement as noted on page 1 of this Agreement. ii. Minor changes in the Work Statement, provided the overall scope of work for the project is unchanged; iii. Modifications to terms consistent with any changes required by Federal, State, or County regulations ordinances or policies; and iv. Modifications to administrative requirements such as changes in reporting periods, frequency of reports, or report formats required by HUD or local regulations, policies or requirements. IGA WITH THE CITY OF EL MIRAGE FOR COMMUNITY DEVELOPMENT BLOCK GRANT ACTIVITIES Approve an Intergovernmental Agreement (“Agreement”) between the City of El Mirage (hereinafter referred to as the “Subrecipient”) and Maricopa County the (“County”) administered by its Human Services Department. The purpose of the Agreement is for the Subrecipient to replace the city’s water lines in the downtown area of El Mirage between Palm and Alto Streets. The County will provide the Subrecipient with U.S. Department of Housing and Urban Development (HUD) Community Development Block Grant (CDBG) funds in the amount not-to-exceed $425,000. Agreement term shall begin November 1, 2019 and will expire November 1, 2021. Also request authorization for the Human Services Department Director to approve and execute Change Orders/Administrative Changes as listed below: Pursuant to Section II Special Provisions paragraph 11.0 ADDITIONAL CERTIFICATIONS AND WARRANTIES subparagraph 11.1.18. TERMINATION OF SUBRECIPIENT AGREEMENT WITH REBUILDING TOGETHER VALLEY OF THE SUN FOR COMMUNITY DEVELOPMENT BLOCK GRANT ACTIVITIES Accept the request to terminate the Subrecipient Agreement between Rebuilding Together, Valley of the Sun (referred to as the “Subrecipient”) and Maricopa County (the “County”) administered through its Human Services Department. The Subrecipient and County are collectively referred to herein as the “Parties” and individually as a “Party.” Subrecipient provided formal notification of their desire to terminate the Subrecipient Agreement. Pursuant to the General Provisions Section I paragraph ‘E’ Termination, subparagraph ‘2’ of the Agreement, Subrecipient hereby terminates the Agreement on the sixtieth (60) day from the date of the Termination letter (August 29, 2019), therefore the Termination effective date shall be October 28, 2019. The Subrecipient states they were unable to fulfill their contractual obligations due to operational issues. The Agreement was funded by a grant from the U. S. Department of Housing and Urban Development (HUD), Community Development Block Grant (CDBG) (C-22-16-032-G-). The term of the Agreement began on September 19, 2018 through June 30, 2020. The County allocated $25,000 for program activities. The Subrecipient did not expend any funds under this Agreement. This Agreement does not impact the General Fund. The engineering design and installation of the work shall meet Maricopa Association of Governments (MAG) Standards and locally adopted codes. The purpose of the Agreement is for the Subrecipient to provide temporary accommodations for COVID positive residents as well as purchase supplies for meal preparation, storage, and delivery for vulnerable and senior residents of Guadalupe, AZ. The supplies will include food, containers, personal protective equipment, and cleaning supplies. The County will provide the Subrecipient with U.S. Department of Housing and Urban Development (HUD) Community Development Block Grant (CDBG) funds authorized by the CARES Act in the amount not-to-exceed $26,216.00. The COVID-19 pandemic has increased the cost of delivery of essential public services, in a safe manner, to vulnerable residents of Guadalupe in two key areas: 1) Community Action Program (CAP) food distribution and 2) Senior Center home delivery and daily “to-go” meals to our Elders. The demand for food distribution has increased, and the senior meals can no longer be served at the Senior Center. Agreement term shall begin July 1, 2020 and will expire July 1, 2021.
