51. COLLABORATION AGREEMENT WITH KEOGH HEALTH CONNECTION Approve a non-financial Collaboration Agreement between Keogh Health Connection, a community based-organization and Maricopa County administered by its Human Services Department. The purpose of the Agreement is for the County to refer individuals to Keogh for information, eligibility determination for the Affordable Care Act. Keogh will utilize the Head Start office located at 2150-1 South Country Club Road, Suite 7, Mesa, AZ to meet with parents of the Head Start program participants. The term of the Agreement is November 1, 2016 and will expire on October 31, 2020 and will be effective upon execution by both Parties. The Agreement may be extended for two (2) additional two-year term The Agreement may be terminated by either Party upon thirty (30) days prior written notice to the other party. Such termination shall be effective upon the expiration of the thirty (30) days unless otherwise agreed to in writing by the parties through a written Amendment. This Agreement does not impact the County General Fund. (C-22-17-026-3-00)
