SOLE SOURCE CONTRACT WITH LODESTAR DAY RESOURCE CENTER Approve a Sole Source and a Contract between Lodestar Day Resource Center (LDRC), a non-profit community based organization, and Maricopa County administered by its Human Services Department. The resulting contract is pursuant to a Sole Source submitted to the Office of Procurement Services. The purpose of this Contract is to provide funds to LDRC for personal baggage storage service individuals using overflow shelter services at the Human Services Campus. The contract term is retroactive beginning July 1, 2017 and will expire on December 31, 2017. The contract amount shall not exceed $33,000. Funding for this contract will be reimbursed through the County General Fund. Supervisory District: 5 (C-22-18-003-3-00)

AMENDMENT TO THE SOLE SOURCE CONTRACT WITH LODESTAR DAY RESOURCE CENTER Approve amendment No. 1 to the Sole Source Contract between Lodestar Day Resource Center (LDRC), a non-profit community based organization, and Maricopa County administered by its Human Services Department. The purpose of the Amendment is to address the following: I. Extend the contract expiration date in paragraph 3.1 from December 31, 2017 through March 31, 2018. II. Increase contract amount in paragraph 7.1 by $16,500 for overflow shelter services. Reimbursement requests will be distributed monthly to be billed through March 31, 2018. III. Parties agree that the statutory language of A.R.S. §38-511 are applicable to this Amendment and incorporate the same by reference. IV. The foregoing paragraphs contain all the changes to the Agreement as amended. All other terms of the original Agreement and Amendments not amended hereby, remain in full force and effect as approved and amended. The purpose of Amendment No. 2 is to address the following: I. Extend the contract expiration date in paragraph 3.1 from March 31, 2018 to June 30, 2018. II. Increase contract amount in paragraph 7.1 by $16,500 for baggage storage services. Reimbursement requests will be distributed monthly to be billed through June 30, 2018. III. Parties agree that the statutory language of A.R.S. §38-511 are applicable to this Amendment and incorporate the same by reference. IV. The foregoing paragraphs contain all the changes to the Agreement as amended. All other terms of the original Agreement and Amendments not amended hereby, remain in full force and effect as approved and amended.
