36. ADMINISTRATIVE CORRECTION TO ADD MODULAR BUILDINGS TO MARICOPA COUNTY ASSET INVENTORY Approve an Administrative Correction to add a Modular Building to Maricopa County asset inventory list. On or about May 1, 1996 Maricopa County entered into a financial agreement with Glendale Elementary School District No. 40 (C22960224) for the acquisition of a modular building serial numbers C-85-2-13564C and C-85-13567C for the amount of $40,000. The agreement included building improvements and installation at the Discovery School located at 7900 W. Maryland, Glendale, AZ. Through an administrative error, the modular building was not added to the County asset list. The building was purchased with federal grant funds provided to the Head Start program to provide services to Head Start program participants. The modular building to be added to the inventory list are: VIN Title # AZ357193 250M017200005 AZ357194 250M017200006 While conducting an inventory of Head Start grant purchases, it was determined that new titles needed to be obtained due to titles associated with the modular building were assigned to another modular building structure. The Arizona Department of Transportation Motor Vehicle Division issued new titles and VIN on July 19, 2017. Approval of this action will authorize Equipment Services to add the modular building to the asset inventory. (C-22-18-016-V-00)

36. TRANSFER MODULAR BUILDING TO CATHOLIC CHARITIES COMMUNITY SERVICES, INC Approve the transfer of a Modular Building from Maricopa County to Catholic Charities Community Services, Inc. (CCCS), a non-profit, community-based organization. Maricopa County purchased the modular building on or about May 1996 with U.S. Department of Health and Human Services (DHHS)/Administration for Children and Families (ACF), Office of Head Start (OHS) funds, provided to Maricopa County. In a letter received by Maricopa County from DHHS, Grant Corporative Agreement Number: 09CH7096/50, OHS “may exercise our right to transfer title of property to a third part, as provided in 2 CRF 75.3018, Real Property, and 75.32, Equipment.” The identified third party is CCCS. As instructed, Maricopa County must transfer the modular building to CCCS. CCCS is now a direct recipient of Head Start grant funds and is no longer a delegate agency for Maricopa County Head Start program. Approval of this item will remove the modular building from the County’s inventory list and will transfer the titles of the modular building to CCCS. The modular building to be transferred is: VIN Title # AZ357193 250M017200005 AZ357194 250M017200006 THIS ITEM REQUIRES UNANIMOUS APPROVAL OF THE BOARD The modular building was purchased through an Agreement with Glendale Elementary School District No. 40 (C22960224), approved by the Maricopa County Board of Supervisors on April 23, 1996. The modular building has been located at the Discovery School located at 7910 W. Maryland, Glendale AZ. As a long term Maricopa County Head Start program contractor, CCCS has utilized the modular building for delivery of activities in western Maricopa County. Supervisor District: 4 (C-22-18-016-V-01)
