30. AGREEMENT AND RECEIPT OF GRANT FUNDS FROM VITALYST HEALTH FOUNDATION FOR HAND-IN-HAND PROJECT TO ASSIST HOMELESS INDIVIDUALS Approve an Agreement and the receipt of grant funds from Vitalyst Health Foundation (Vitalyst), a nonprofit corporation. Vitalyst will provide the County with $60,000 in grant funds for the Hand-in-Hand Project to work with supportive services, housing providers, physical and mental health services, jails, and policy makers to serve justice-involved homeless individuals and families; by connecting them with necessary support and housing services. The funding period is retroactive to July 1, 2018 through June 30, 2019. The Maricopa County Human Services Department and several government agencies and community based organizations have created a partnership to reduce recidivism, and connect people experiencing homelessness who are justice engaged, to appropriate housing, and supportive services. The agencies include: Maricopa County Correctional Health Services, (CHS), Justice Systems Planning & Information (JSPI), Housing Authority of Maricopa County (HAMC), Mercy Maricopa Integrated Care (MMIC), Native American Connections (NAC), Community Bridges, Inc. (CBI), and Arizona Behavioral Health Corporation (ABC). The Vitalyst Health Foundation Board of Directors awarded the County with grant funds to support the services through the Hand-in-Hand project. The grant is a one-time grant and does not require a cash or in-kind match. The receipt of the funds does not require future or ongoing contributions. The Hand-in-Hand project activities are some of the same activities that the Human Services Department, Housing and Community Development currently work on with community partners. Even though a match or in-kind is not required, the Department has provided a budget for the activities which outlines the Department’s commitment towards this project. The Human Services Department FY2019 Indirect cost rate has not been fully negotiated with the U.S. Department of Health and Human Services. The Department’s FY2019 indirect cost rate is 22.9% for salaries and employee related expenses. The total grant funds are $60,000 of which $0 is for salaries and benefits. The total estimated indirect costs are $0 and $0 is recoverable. The grant award is not a mandated function but provides a benefit to the citizens through working hand in hand with supportive services, housing providers, physical and mental health services, jails, and policy makers to serve justice-involved homeless individuals and families; by connecting them with necessary supports and housing. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and Therefore, expenditure of the revenues is not prohibited by the budget law. This Agreement does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. 42-17105. The overall grant budget will be adjusted as necessary to accommodate the Agreement funding through a future reconciliation. Acceptance of the grant funds will not impact the County General Fund. The Vitalyst Foundation Board of Directors made a direct allocation of funds through a non-competitive process to support the County’s Hand in hand program. 43. AGREEMENT WITH COMMUNITY BRIDGES, INC. Approve a financial Agreement between Community Bridges, Inc., (CBI) a community based organization and Maricopa County (County) administered by its Human Services Department (HSD). The purpose of this Agreement is for CBI to provide navigation and coordination services to homeless individuals in the County’s Hand in Hand Program. CBI shall provide service delivery using progressive engagement strategies designed to help participants meet their individual goals for self-sufficiency and self-determination. The services will provide opportunities for Participants to access a full array of services and programs that will meet their specific needs while encouraging housing stability and independent living. The Agreement shall be retroactive to September 1, 2018 through June 30, 2019. The County shall provide CBI with $54,135 for the Agreement term period. The Agreement may be extended for three (3) additional one (1) year terms upon acceptance and approval by the Parties. There is not a long-term commitment on the part of the County beyond the terms of this Agreement. Maricopa County received a grant from the Vitalyst Health Foundation (C-22-19-003-G-00) to support the efforts of the Hand in Hand program. CBI was expressly written into the Vitalyst Health Foundation grant request as the service provider for these funds as this was a collaborative application where Maricopa County Human Services Department administers the grant activities. Therefore, HSD requested and received approval for a Competition Impracticable from the Office of Procurement Services for the Agreement with CBI. Supervisory District: All (C-22-19-010-3-00)
