35. AMENDMENT TO THE AGREEMENT WITH HUMAN SERVICES CAMPUS, INC. FOR SHELTER SERVICES Approve financial Amendment No. 3 to the Agreement between Human Services Campus, Inc., (“Contractor”) and Maricopa County administered by its Human Services Department. The Parties agree to enter into this Amendment No. 3 to the Agreement to amend the Agreement as follows: A. Extend the Agreement term from June 30, 2020 to October 31, 2020. B. Revise Paragraph 9.0 (Contractor Responsibilities) to add the following: 9.4 The Subrecipient shall provide the following Services to ensure homeless individuals utilizing the County lots, located on 9th Avenue between Jefferson Avenue and Madison Street. in Phoenix are in a safe environment: 9.4.1 Staff to oversee service activities; and 9.4.2 Three (3) Armed and/or Unarmed Security Guards to patrol the perimeter of the County lots. 9.5 The Services begin on April 20, 2020 and extend through May 20, 2020. Therefore, the Services shall continue on a month-to-month basis if the County provides the Subrecipient with notice at least five (5) business days prior to the 20th day of each month. The written notice for continuation of the Services shall be provided by means of a written Administrative Change Order executed by the Human Services Department Director which shall be approved and signed by the Subrecipient. C. The County shall provide the Subrecipient with additional funding in an amount not-to-exceed $219,762 for the Services identified in Paragraphs 9.4 and 9.5. The maximum monthly billing amount shall not exceed $73,254. D. Add the following paragraph: 35.0 ADMINISTRATIVE CHANGE ORDERS 35.1 The Human Services Department Director may make changes within the general scope of the Agreement on behalf of the County through Administrative Change Orders approved and fully executed by the Parties. 35.2 Administrative Change Orders may modify the timeline of the activities if the last day of the timeline is within the Agreement Term. The County shall reimburse the Subrecipient on a Net 0 payment basis. This Agreement is subject to A.R.S. §38-511. The foregoing paragraphs contain all the changes made by this Amendment No. 3. All other terms and conditions of the original Agreement and Amendments No. 1 and Amendment No. 2 shall remain the same and in full force and effect as approved and amended. Also request approval and authorization for the Human Services Department Director to execute Administrative Change Orders which extend the Services identified in section 9.4 and 9.5. The Administrative Change Orders will be reviewed and approved by the Human Services Department appointed Attorney and fully executed Administrative Change Orders will be provided to the Maricopa County Clerk of the Board’s office for filing and incorporated into the Agreement. The County and the Human Services Campus, Inc., entered into a Contract on or about January 1, 2019, for the provision of emergency weather relief accommodations for individuals experiencing homelessness at the Human Services Campus. Office of Procurement Services recommend a Competition Impracticable because the Contractor is the only low-barrier shelter services providers in downtown Phoenix that provide temporary weather relief services. 48. AMENDMENT TO AGREEMENT WITH HUMAN SERVICES CAMPUS, INC. FOR SHELTER SERVICES Approve financial Amendment No. 4 to the Agreement between Human Services Campus, Inc., (“Subrecipient”) and Maricopa County administered by its Human Services Department. The Parties agree to enter into this Amendment No. 4 to the Agreement to amend the Agreement as follows: A. Revise Paragraph 9.4.2 to read as follows: 9.4.2. Three (3) Armed and/or Unarmed Security Guards to patrol the County lots 24 hours a day, 7 days a week and one (1) additional Armed or Unarmed Security Guard to patrol the County lots in the evening for 12 hours, 7 days a week. B. Add the following new paragraphs: 9.1.1 The Contractor shall purchase the following items for the provision of weather relief services to serve those experiencing homelessness due to reduced indoor capacity because of COVID-19 (18) evaporative coolers; canopies to cover (4) areas on the property; and awnings to use in (4) areas on the property. 9.1.1.2 For any items considered capital assets as defined by Uniform Administrative Requirements, the Contractor must adhere to the appropriate procurement and monitoring as required by Uniform Administrative Requirements. 36.0 WRITTEN CERTIFICATION PURSUANT TO A.R.S. § 35-393.01 If the Subrecipient engages in for-profit activity and has 10 or more employees, and if this Agreement has a value of $100,000 or more, then the Subrecipient certifies it is not currently engaged in, and agrees for the duration of this Agreement not to engage in, a boycott of goods and services from Israel. This certification does not apply to a boycott prohibited by 50 U.S.C. § 4842 or a regulation issued pursuant to 50 U.S.C. § 4842. 37.0 UNIFORM ADMINISTRATIVE REQUIREMENTS The Subrecipient agrees to comply with all applicable provisions of Title 2, Subtitle A, Chapter II, Part 200—UNIFORM ADMINISTRATIVE REQUIREMENTS, COST PRINCIPLES, AND AUDIT REQUIREMENTS FOR FEDERAL AWARDS contained in Title 2 C.F.R. § 200, et seq. C. Increase the County’s funding to the Subrecipient for the items identified in paragraph 9.1.1 to a not to exceed amount of $92,200. Funding availability is from June 10, 2020 through August 31, 2020. D. Replace Amendment III C. with the following: i. The County shall provide the Subrecipient with additional funding in an amount not to exceed $269,097 for the Services identified in Paragraph 9.4 and 9.5. The maximum monthly billing amount not to exceed $89,699. Funding availability is from April 20, 2020 through October 31, 2020. This Amendment No. 4 is subject to the provisions of A.R.S. § 38-511. The foregoing paragraphs contain all the changes made by this Amendment No.4. All other terms and conditions of the Agreement and Amendment Nos. 1, 2, and 3 shall remain the same and in full force and effect as approved and amended. The Parties have authorized the undersigned to execute this Amendment No. 4 on their behalf. This Amendment No. 4 shall be effective upon approval and signature by both Parties. (C-22-19-020-3-04)

FOR SHELTER SERVICES Approve financial Amendment No. 6 to the Agreement between Human Services Campus, Inc., (“Subrecipient”) and Maricopa County administered by its Human Services Department. The County and the Subrecipient are referred to collectively as the “Parties.” The Parties agree to enter into this Amendment No. 6 to the Agreement to address the following: 1. Extend the Agreement expiration date from October 31, 2020 through December 30, 2020. 2. The County shall provide the Subrecipient with funding for continued operation of the County lots, located on 9th Avenue between Jefferson Avenue and Madison Street in Phoenix (Safe Outdoor Space (SOS)). 30. AMENDMENT TO THE AGREEMENT WITH HUMAN SERVICES CAMPUS, INC. FOR SHELTER SERVICES Approve financial Amendment No. 7 to the Agreement between Maricopa County (“County”), administered by its Human Services Department, and Human Services Campus, Inc., (“Subrecipient”). The County and the Subrecipient are referred to collectively as the “Parties.” The Parties agree to enter into this Amendment No. 7 to amend the Agreement as follows: A. Add that the County shall provide the Subrecipient an amount not to exceed $159,000 for the services identified in Subparagraph 9.6 of the Agreement. The funding is provided under the Coronavirus Aid, Relief, and Economic Security Act (CARES Act), Public Law 116-136 (Catalog of Federal Domestic Assistance (CFDA) 21.019) The funding availability period is from October 1, 2020, through December 30, 2020. Monthly expenditure amounts shall be limited to $53,000. B. The Contractor shall ensure all activities related to installation of the (18) evaporative coolers; canopies to cover (4) areas on the property; and awnings to use in (4) areas on the property, as identified in Sub-subparagraph 9.1.1 of the Agreement, shall be completed by December 30, 2020. The Contractor shall submit invoices no later than the 10th business day of January 2021. C. Add that the County shall provide the Subrecipient an amount not to exceed $30,000 for the cleaning services for the County lots located on 9th Avenue between Jefferson Avenue and Madison Street in Phoenix (Safe Outdoor Space). Funding for lot cleaning shall not exceed $10,000 per month. D. The Amendment total shall not exceed $189,000. The foregoing paragraphs contain all the changes made by this Amendment No.7. All other terms and conditions of the Agreement and Amendment Nos. 1, 2, 3, 4, 5 and 6 shall remain the same and in full force and effect as approved and amended. Extend the Agreement termination date from September 30, 2021 through December 31, 2021. B. The County shall provide the Subrecipient with: i . $221,232 for Covid-19 weather relief services at the Human Services Campus overflow shelter. The funding for such services under this Amendment No. 13 shall be provided by Maricopa County General Funds. 44. AMENDMENT TO THE AGREEMENT WITH HUMAN SERVICES CAMPUS, INC. FOR SHELTER SERVICES Approve financial Amendment No. 14 to the Agreement between Maricopa County (“County”), administered by its Human Services Department, and Human Services Campus, Inc., (“Subrecipient”). The County and the Subrecipient are referred to collectively as the “Parties.” The Parties agree to enter into this Amendment No. 14 to amend the Agreement as follows: 1. Extend the Agreement termination date retroactively from January 1, 2022 through June 30, 2022. 2. The County shall provide the Subrecipient with: 2.1 $472,784 for the Subrecipient to continue emergency weather relief services at the Human Services Campus overflow shelter, identified in the attached Human Services Campus Overflow Shelter Budget. The funding for the services shall be provided by Maricopa County General Funds. 56. AMENDMENT TO THE AGREEMENT WITH HUMAN SERVICES CAMPUS, INC. FOR SHELTER SERVICES Approve financial Amendment No. 15 to the Agreement between Maricopa County (“County”), administered by its Human Services Department, and Human Services Campus, Inc., (“Subrecipient”). The County shall provide the Subrecipient with a total of $83,000 in funding for Amendment No. 15, to provide drinking water to homeless people who will utilize the heat relief shelter during the summer months. 2.
