25. DATA SHARING AGREEMENT WITH ARIZONA DEPARTMENT OF HEALTH SERVICES OFFICE OF ENVIRONMENTAL HEALTH Approve a non-financial Data Sharing Agreement (DSA) between Arizona Department of Health Services Office of Environmental Health (ADHS) and Maricopa County, administered by its Human Services Department. Through this Agreement, ADHS and the Human Services Department, Housing and Community Development Division (HCD) will share data regarding children identified with elevated blood lead levels that reside in the County that have an exposure to suspected lead-based paint source. The following data will be shared: • blood lead testing results, • environmental testing results, • remediation activities for cases referred by ADHS, and • summary of program activities conducted to ensure a child with elevated blood lead levels receives appropriate interventions and case management services The Agreement will be effective upon signature of both parties and may only be terminated upon sixty (60) calendar day’s written notice to the other party. ADHS will provide test results information to HCD on a monthly basis. Supervisory District: All (C-22-20-018-3-00)
