33. AMENDMENT TO THE AGREEMENT WITH COMMUNITY BRIDGES, INC. FOR TEMPORARY HOMELESS SHELTER OPERATIONS Approve financial Amendment No. 1 to the Agreement between Community Bridges, Inc. (CBI), a community-based organization and Maricopa County administered by its Human Services Department. The purpose of the Amendment is to address the following: A. Revise Section 3 (Work Statement), Paragraph 1.0 (Scope of Work), Subparagraph 1.1 (Facility) as follows: 1.1.2 The services shall be provided at that the following locations: 1.1.2.1 A New Foundation 1200 North 77th Street, Scottsdale, Arizona 85257; and 1.1.2.2 Travel Lodge 600 West Van Buren Street, Phoenix, Arizona 85003: 1.1.2.2.1 Services to begin April 27, 2020 and extend through May 27, 2020. Therefore, the Services shall continue on a month-to-month basis if the County provides the Subrecipient with notice at least five (5) business days prior to the 27th day of each month. The written notice for continuation of the Services shall be provided by means of a written Administrative Change Order executed by the Human Services Department Director which shall be approved and signed by the Subrecipient. B. Revise Section 3 (Work Statement), Paragraph 1.0 (Scope of Work), to add the following Subparagraph: 1.9 Oversight and Supervision 1.9.1 The Subrecipient shall provide oversight and supervision for the following individuals and their work activities: 1.9.1.1 Security Guards: 1.9.1.1.1 Monitor work activities; 1.9.1.1.2 Provide direction; and 1.9.1.1.3 Approve timecards. C. Revise Attachment A (CBI Line item Budget – 6 month Contract) to include budget costs for the services to be provided at the locations identified in paragraph 1.1.2 (Facility) and replace it with the Revised Attachment A attached to this Amendment No. 1. D. The County will provide the Subrecipient with additional funding in an amount not-exceed amount $79,305 for operation of the temporary shelter services to be provided at the Travel Lodge location, 600 West Van Buren Street, Phoenix, Arizona 85003. The maximum monthly billing amount shall not exceed $26,435. E. The total Agreement amount now will be increased from $295,296 to a not-to-exceed amount of $374,601. Funding for this Agreement is provided by U.S. Department of Housing and Community Development (HUD), Community Development Block Grant (CDBG) and Emergency Solutions Grant (ESG) funds through the “Coronavirus Aid, Relief, and Economic Security Act” (CARES Act). 60. AMENDMENT TO THE AGREEMENT WITH COMMUNITY BRIDGES, INC. FOR TEMPORARY HOMELESS SHELTER OPERATIONS Approve financial Amendment No. 2 to the Agreement between Community Bridges, Inc. (CBI), a community-based organization and Maricopa County administered by its Human Services Department. The purpose of the Amendment is to address the following: A. Revise and replace the Section 1 (General Provisions), Paragraph 1.0 (Purpose) as follows: The County shall provide the Subrecipient with Coronavirus Relief Funds from the Coronavirus Aid, Relief, and Economic Security Act (CARES Act) for the provision of activities in Section 3 (Work Statement). B. Remove references to the U.S. Department of Housing and Urban Development, Community Development Block Grant (CDBG), and Emergency Solutions Grant (ESG). C. Revise Section 1 (General Provisions) to add the following new section: 53.0 WRITTEN CERTIFICATION PURSUANT TO A.R.S. § 35-393.01 If the Subrecipient engages in for-profit activity and has 10 or more employees, and if this Agreement has a value of $100,000 or more, then the Subrecipient certifies it is not currently engaged in, and agrees for the duration of this Agreement not to engage in, a boycott of goods and services from Israel. This certification does not apply to a boycott prohibited by 50 U.S.C. § 4842 or a regulation issued pursuant to 50 U.S.C. § 4842. D. Revise Section 2 (Special Provisions), Paragraph 4.3 (Budget Adjustments, Change Orders and Amendments), to add the following Subparagraph: 4.3.4 Continuance of services on a month-to-month basis at the Travel Lodge and Sure Stay locations provided the extensions do not extend past the Current Agreement Termination Date. E. Revise Section 3 (Work Statement), Paragraph 1.0 (Scope of Work), Subparagraph 1.1 (Facility) as follows: 1.1.2 The services shall be provided at that the following locations: 1.1.2.1 A New Foundation 1200 North 77th Street, Scottsdale, Arizona 85257; and 1.1.2.2 Travel Lodge 600 West Van Buren Street, Phoenix, Arizona 85003: 1.1.2.2.1 Services to begin April 27, 2020 and extend through June 21, 2020. Thereafter, the Services shall continue on a month-to-month basis if the County provides the Subrecipient with notice at least five (5) business days prior to the 27th day of each month. However, the Services shall not extend past the current Termination date (i.e., October 8, 2020). The written notice for continuation of the Services shall be provided by means of a written Administrative Change Order executed by the Human Services Department Director and by the Subrecipient and approved by counsel. 1.1.2.3 Sure Stay 424 West Van Buren Street, Phoenix, Arizona 85003: 1.1.2.3.1 Services to begin May 13, 2020 and extend through July 17, 2020. Thereafter, the Services shall continue on a month-to-month basis if the County provides the Subrecipient with notice at least five (5) business days prior to the 17th day of each month. However, the Service shall not extend past the current Termination date (i.e., October 8, 2020). The written notice for continuation of the Services shall be provided by means of a written Administrative Change Order executed by the Human Services Department Director and by the Subrecipient and approved by counsel. 1.1.2.4 Phoenix Inn 8152 N Black Canyon Hwy, Phoenix, AZ 85051: 1.1.2.4.1 Services to begin June 22, 2020 and extend through July 30, 2020. Thereafter, the Services shall continue on a month-to-month basis if the County provides the Subrecipient with notice at least five (5) business days prior to the 17th day of each month. However, the Service shall not extend past the current Termination date (i.e., October 8, 2020). The written notice for continuation of the Services shall be provided by means of a written Administrative Change Order executed by the Human Services Department Director and by the Subrecipient and approved by counsel. F. Revise Section 3 (Work Statement), Paragraph 1.0 (Scope of Work), Subparagraph 1.3 (Operations) to add the following: 1.3.1 The Subrecipient shall: 1.3.1.6 Maintain an unusual incident log and contact the County via email and phone within two (2) hours of any unusual incident that requires escalation per the Subrecipient’s internal policy and procedures. G. Revise Attachment A (CBI Line item Budget – 6 month Contract) to include budget costs for the services to be provided at the locations identified in paragraph 1.1.2 (Facility) and replace it with the Revised Attachment A attached to this Amendment No. 2. H. Add that the County will provide the Subrecipient with additional funding for in a not-to-exceed amount for operation of the temporary shelter services identified in 1.1.2. by $96,887. The maximum billing amount shall not exceed $116,630. I. Add that the total Agreement amount shall now will be increased by $96,887 from $374,601 to a not-to-exceed amount of $471,488. The foregoing paragraphs contain all the changes to the Agreement made by this Amendment No. 2. All other terms and conditions of the Agreement and Amendment No. 1 shall remain unchanged and in full force and effect as executed by the Parties. Funding for this Agreement is provided by Coronavirus Relief Funds from the Coronavirus Aid, Relief, and Economic Security Act (CARES Act) received by Maricopa County. Extend the term of the Agreement from October 8, 2020 through December 31, 2020; B. The County shall increase funding by a not-to-exceed amount of $656,292, the period of funding availability shall be from July 1, 2020, through December 31, 2020. Monthly invoices shall not exceed $109,382 (Revised Attachment A). The Agreement total shall be increased from $471,488 to $1,127,780; C. Remove the Travel Lodge 600 West Van Buren Street from the Agreement as a service location as of June 30, 2020; D. End the A New Foundation 1200 North 77th Street, Scottsdale location as a service location as of September 30, 2020; E. Amend Section 2 (Special Provisions), Paragraph 4.0 (Budget Adjustments, Change Orders and Amendments), Subparagraph 4.3 and shall provide the Subrecipient with a Written Change Order identifying the new service location: 4.3.4 Identify at a later date a new service location when the Parties previously have agreed through a written amendment to create a new service location; and 4.3.5 Create a new service location, with the address to be later identified by the Parties through the change order process set forth in Section 2 (Special Provisions), Paragraph 4.0 (Budget Adjustments, Change Orders and Amendments). The foregoing paragraphs contain all the changes to the Agreement made by this Amendment No. 3. All other terms and conditions of the Agreement and Amendment Nos. 1, and 2 shall remain unchanged and in full force and effect as executed by the Parties. (Subrecipient), a community-based organization and Maricopa County (County) administered by its Human Services Department. The purpose of the Agreement is for the operation of temporary emergency shelter services for homeless individuals experiencing homelessness in Maricopa County in an effort to protect them from exposure to COVID-19. The Subrecipient and the County shall be referred to as the “Parties.” The purpose of the Amendment is to address the following: A. Extend the term of the Agreement from October 1, 2021 through December 31, 2021. B. Add that the County shall increase funding by a not-to-exceed amount of $328,146. Funding availability period shall begin on October 1, 2021 through December 31, 2021. The Agreement total shall be increased from $2,768,510 to a not-to-exceed amount of $3,096,656. Funding for Amendment No. 7 is provided by Catalog of Federal Domestic Assistance (CFDA) 81.135 American Rescue Plan Act (ARPA). Monthly invoices shall not exceed $109,382. C. Revise Section 1 (General Provisions) to address the following: 1. 43. AMENDMENT TO THE AGREEMENT WITH COMMUNITY BRIDGES, INC. FOR TEMPORARY HOMELESS SHELTER OPERATIONS Approve financial Amendment No. 8 to the Agreement between Community Bridges, Inc. (Subrecipient), a community-based organization and Maricopa County (County) administered by its Human Services Department. The purpose of the Agreement is for the operation of temporary emergency shelter services for homeless individuals experiencing homelessness in Maricopa County in an effort to protect them from exposure to COVID-19. The Subrecipient and the County shall be referred to as the “Parties.” The purpose of the Amendment is to address the following: 1. Correct the funding referenced in Amendment 7. The Amendment incorrectly listed CFDA No. 81.135. The correct CFDA No. is 21,027. 2. Extend the term of the Agreement retroactively from January 1, 2022 through June 30, 2022. 3. Add that the County shall increase funding by a not-to-exceed amount of $530,862. Funding availability period shall begin retroactively on January 1, 2022 through June 30, 2022. The Subrecipient and the County shall be referred to as the “Parties.” The purpose of the Amendment is to address the following: 1. The purpose of the Agreement is for the Subrecipient to provide operation of emergency shelter services for individuals experiencing homelessness in Maricopa County including providing for basic needs, case management, and housing referrals at the 424 W. Van Buren Street location. The County has provided the Subrecipient with $8,630,327.78 for the shelter services. The Agreement term is April 8, 2020, through December 31, 2024. The purpose of the Amendment is to address the following: A. Decrease funds by $407,924 from $8,630,327.78 to a new total amount of $8,222,403.78. B. Update Point of Contact for the County C.
