43. AMENDMENT TO THE AGREEMENT WITH COMMUNITY BRIDGES, INC. FOR TEMPORARY HOMELESS SHELTER OPERATIONS Approve financial Amendment No. 8 to the Agreement between Community Bridges, Inc. (Subrecipient), a community-based organization and Maricopa County (County) administered by its Human Services Department. The purpose of the Agreement is for the operation of temporary emergency shelter services for homeless individuals experiencing homelessness in Maricopa County in an effort to protect them from exposure to COVID-19. The Subrecipient and the County shall be referred to as the “Parties.” The purpose of the Amendment is to address the following: 1. Correct the funding referenced in Amendment 7. The Amendment incorrectly listed CFDA No. 81.135. The correct CFDA No. is 21,027. 2. Extend the term of the Agreement retroactively from January 1, 2022 through June 30, 2022. 3. Add that the County shall increase funding by a not-to-exceed amount of $530,862. Funding availability period shall begin retroactively on January 1, 2022 through June 30, 2022. The Agreement total shall be increased from $3,096,656 to a not-to-exceed amount of $3,627,518. Funding for Amendment No. 8 is provided under CFDA number 21.027 American Rescue Plan Act, Coronavirus State and Local Fiscal Recovery Funds (CSLFRF). Monthly invoices shall not exceed $88,477. 4. Revise Section 4 (Compensation) to address the Reimbursement process. All other terms and conditions of the Agreement and previously approved, Amendments and Administrative Change Order No. 1 not amended by this Amendment No. 8 shall remain unchanged and in full force and effect as executed by the Parties. 60. AMENDMENT TO AGREEMENT WITH COMMUNITY BRIDGES, INC. FOR TEMPORARY HOMELESS SHELTER OPERATIONS Approve financial Amendment No. 9 to the Agreement between Community Bridges, Inc. (Subrecipient), a community-based organization and Maricopa County (County) administered by its Human Services Department. The purpose of the Agreement is for the operation of temporary emergency shelter services for homeless individuals experiencing homel essness in Maricopa County. The Subrecipient and the County shall be referred to as the “Parties.” The purpose of the Amendment is to address the following: 1. Extend the term of the Agreement from July 1, 2022 through June 30, 2024. 2. The County shall County shall provide Subrecipient with a not-to-exceed amount of $5,073,503.78, ($2,536,751.89 annually). Funding availability period shall begin on July 1, 2022 through June 30, 2024. Funding for Amendment No. 9 is provided under Assistance Listing Numbe r (ALN) 21.027 American Rescue Plan Act, Coronavirus State and Local Fiscal Recovery Funds (CSLFRF). Monthly invoices shall not exceed $211,396. 3. Revise Section 3 (Work Statement) Paragraph 1.0 (Scope of Work) to remove in its entirety and replace with a revised Scope of Work. In the revised Scope of Work, the Subrecipient will continue providing temporary emergency shelter services, outreach and case management to individuals experiencing homelessness in the Phoenix metropolitan area. 4. Revise Section IV (Compensation) to address the Reimbursement process. 5. The Agreement total shall be increased from $3,627,518 to a not-to-exceed amount of $8,701,021.78 The Agreement is amended to incorporate the changes contained in this Amendment No. 9. All other terms and conditions of the Agreement and previously approved Amendments and Administrative Change Order No. 1 not amended by this Amendment No. 9 shall remain unchanged and in full force and effect as executed by the Parties. The Subrecipient and the County shall be referred to as the “Parties.” The purpose of the Amendment is to address the following: 1. Revise the fund sources for $68,563 of the $401,264 provided in Amendment No. 10. The total Agreement funding amount remains unchanged. The changes include: a. Reduce Emergency Solutions Grant-CV funds by $68,563 to a new total of $332,701; and b. Add $68,563 in ARPA funds under ALN 21.027. The Agreement total remains unchanged at a total of $8,701,021.78. The Agreement is amended to incorporate the changes contained in this Amendment No. 11. The purpose of the Agreement is for the operation of temporary emergency shelter services for individuals experiencing homelessness in Maricopa County. The Subrecipient and the County shall be referred to as the “Parties.” The purpose of the Amendment is to address the following: A. The County shall provide the Subrecipient with a lump sum of $600,000 in ARPA funds allocated in Amendment No. 9. The Amendment funding amount of $5,073,504 remains unchanged. The Subrecipient shall continue to submit monthly funding expenditure documentation, such as a General Ledger or a detailed Expenditure Report, with the included charges identified. B. Revise the fund sources for $68,563 of the $332,701 provided in Amendment No. 11. The Agreement total funding amount remains unchanged. Changes include: 1. Reduce ARPA funds by $68,563. 2. Add $68,563 in Emergency Solutions Grant-CV to a new total of $401,264 C. This Amendment shall be effective upon approval and execution by both Parties. The Agreement is amended to incorporate the changes contained in this Amendment No. 12. 45. AMENDMENT TO AGREEMENT WITH COMMUNITY BRIDGES, INC. FOR SHELTER SERVICES OPERATIONS Approve financial Amendment No. 15 to the Agreement between Community Bridges, Inc. (Subrecipient), a community-based organization, and Maricopa County (County) administered by its Human Services Department. The purpose of the Agreement is for the Subrecipient to provide operation of emergency shelter services for individuals experiencing homelessness in Maricopa County including providing for basic needs, case management, and housing referrals at the 424 W. Van Buren Street location. The County has provided the Subrecipient with $8,630,327.78 for the shelter services. The Agreement term is April 8, 2020, through December 31, 2024. The purpose of the Amendment is to address the following: A. Decrease funds by $407,924 from $8,630,327.78 to a new total amount of $8,222,403.78. B. Update Point of Contact for the County C.
