47. AMENDMENT TO THE COMPETITION IMPRACTICABLE CONTRACT WITH CIRCLE THE CITY FOR SERVICES TO HOMELESS INDIVIDUALS Approve Amendment No. 7 to the Competition Impracticable Contract (“Agreement”) between Circle the City (“Contractor”) and Maricopa County administered by its Human Services Department (“County”). The purpose of the Amendment is to address the following: 1. Extend the Termination Date, retroactively from December 31, 2021 through June 30, 2022. 2. Revise Section 2.0 (Purpose), by addressing the following: 2.1 Continue to provide medical care and nursing care services (“Services”) to individuals experiencing homelessness who are either COVID-19 positive or symptomatic (or both). Contractor shall provide Services at the following locations: 2.1.1 Travelodge Phoenix Downtown located at 600 W. Van Buren Street, Phoenix, 85003 through December 21, 2021; and 2.1.2 Extend A Suites Hotel owned by North Black Canyon Hospitality, LLC (“Hotel”) located at 8101 North Black Canyon Hwy Phoenix, 85021 from December 20, 2021 through June 30, 2022. 3. Revise Section 10.0 (Funding, Invoicing, and Payment) to address the following: 10.6 The County shall provide the Contractor with additional funding in the amount not to exceed $1,367,000 to continue providing Services at the locations listed in Subparagraph 2.1. The funding availability period is from January 1, 2022 through June 30, 2022. Funding is provided under CFDA number 21.027 American Rescue Plan Act, Coronavirus State and Local Fiscal Recovery Funds (CSLFRF). 10.7 The Agreement amount shall now be increased to a total amount of $6,207,804. 10.8 The Contractor shall submit monthly invoices no later than the 15th calendar day of each month. 10.9 The Contractor shall submit the December 2021 invoice no later than January 15, 2022. 10.10 The June 2022 invoice shall be submitted no later than July 15, 2022. All other terms and conditions of the Agreement and previously approved Amendments not amended by this Amendment No. 7 shall remain unchanged and in full force and effect as executed by the Parties. The Agreement shall become effective upon approval and signature by both Parties. FY2022: $1,367,000 Expenditures. Supervisory District: All (C-22-20-059-3-07)

48. AMENDMENT TO THE COMPETITION IMPRACTICABLE CONTRACT WITH CIRCLE THE CITY FOR SERVICES TO HOMELESS INDIVIDUALS Approve Amendment No. 8 to the Competition Impracticable Contract (“Agreement”) between Circle the City (“Contractor”) and Maricopa County administered by its Human Services Department (“County”). The purpose of the Amendment is to address the following: 1. Revise Section 2.0 (Purpose), by deleting it in its entirety and replace with the following: 2.1 To continue to provide medical care and nursing care services (“Services”) to individuals experiencing homelessness who are either COVID-19 positive or symptomatic (or both). Contractor shall provide Services at the following location: 2.1.1 Extend A Suites Hotel owned by North Black Canyon Hospitality, LLC (“Hotel”) located at 8101 North Black Canyon Hwy Phoenix, 85021 from December 20, 2021 through June 30, 2022. 2.2 The Contractor shall hire an onsite Nurse Supervisor to supervise nursing services to individuals experiencing homelessness at the Covid Isolation hotel identified in 2.1.1 above, through June 30, 2022. 2. Revise Section 10.0 (Funding, Invoicing, and Payment) to address the following: 10.6 The County shall provide the Contractor with additional funding in the amount not to exceed $48,960, representing 50% of the hourly rate for an on-site nurse supervisor listed in Subparagraph 2.2. The funding availability period is from January 1, 2022 through June 30, 2022. Funding is provided under CFDA number 21.027 American Rescue Plan Act, Coronavirus State and Local Fiscal Recovery Funds (CSLFRF). 10.7 The Agreement amount shall now be increased to a total amount of $6,256,764. 10.8 The Contractor shall submit monthly invoices no later than the 15th calendar day of each month. 10.9 The Contractor shall submit the December 2021 invoice no later than January 15, 2022. 10.10 The June 2022 invoice shall be submitted no later than July 15, 2022. All other terms and conditions of the Agreement and previously approved Amendments not amended by this Amendment No. 8 shall remain unchanged and in full force and effect as executed by the Parties.
