65. AMENDMENT TO THE AGREEMENT WITH COMMUNITY BRIDGES, INC. FOR THE PROVISION OF RAPID REHOUSING SERVICES Approve Amendment No. 3 to a financial Agreement (“Agreement”) between Community Bridges, Inc. (“Subrecipient”) and Maricopa County the (“County”) administered by its Human Services Department. The Purpose of this Amendment is to address the following: A. Add that the County shall provide the Subrecipient with an additional $150,000 in ESG Program Year/Fiscal Year (PY2021/FY2022) funding for rapid rehousing services. Upon approval of this Amendment the total funding amount for this Agreement shall be: 1. $300,000 ESG, 2. $400,000 ESG-CV, and 3. $460,000 ESG-CV2. The total available funding under the Agreement now is increased from $1,010,000 to a not-to-exceed amount of $1,160,000. B. Revise the Contract Amount on page 1 of the Agreement to $1,160,000. C. Period of funding availability for Amendment No. 3 funds shall expire on June 30, 2022. D. Regarding Section 4 (Compensation): 1. Replace Paragraph 1.0 (Funding) in its entirety with the following: 1.0 FUNDING 1.1 Funding for this Agreement is through the following federal Catalog of Federal Domestic Assistance (CFDA) programs: $300,000 – 14.231 Emergency Solutions Grant (ESG ); $400,000 – Emergency Solutions CARES Act (ESG-CV); and $460,000 - Emergency Solutions CARES Act (ESG-CV2). 2. Replace Paragraph 3.0 (Budget) in its entirety with the following: 3.0 BUDGET 3.1 The Agreement budget amount shall not exceed $1,160,000. 3.2 The Subrecipient shall be reimbursed for the line items identified in Revised Attachment 1 (ESG Budget). E. Revise Section 5 (Attachments), by replacing it with the attached Revised Attachment 1 ESG Budget. All other terms and conditions of the Agreement and previously approved Amendments shall remain the same and in full force and effect as executed by both Parties. 55. AMENDMENT TO AGREEMENT WITH COMMUNITY BRIDGES, INC. FOR THE PROVISION OF RAPID REHOUSING SERVICES Approve Amendment No. 4 to a financial Agreement (“Agreement”) between Community Bridges, Inc. (“Subrecipient”) and Maricopa County the (“County”) administered by its Human Services Department. The purpose of the Agreement is for the provision of rapid rehousing services for individuals and families expe riencing homelessness in Maricopa County. The County and Subrecipient collectively are referred to her as the “Parties.” The Purpose of this Amendment is to address the following: A. Extend the term of the Agreement from September 30, 2022 through June 30, 2023. B. Add that the County shall provide the Subrecipient with an additional $152,987 in ESG Program Year/Fiscal Year (PY2022/FY2023) funding for rapid rehousing services. Period of funding availability for Amendment No. 4 funds shall expire on June 30, 2023. C. Revise Section 4 (Compensation), Paragraph 1.0 (Funding) and Paragraph 3.0 (Budget) to identify the funding allocation for this Agreement.: D. Revise Section 5 (Attachments), by replacing the Revised Attachment 1 CBI RRH ESG Budget. The Agreement is amended to incorporate the changes contained in this Amendment No. 4. All other terms and conditions of the Agreement and previously approved Amendments shall remain unchanged and in full force and effect as executed by the Parties. Amendment No. 4 shall be effective upon approval and signature by both Parties. FY2023: $152,987 Expenditures Supervisory District: All (C-22-20-061-3-04)
