59. AMENDMENT TO THE DIOCESAN COUNCIL FOR THE SOCIETY OF SAINT VINCENT DE PAUL AGREEMENT FOR TEMPORARY HOMELESS SHELTER Approve Amendment No. 3 to the Agreement between Diocesan Council for the Society of Saint Vincent de Paul (Contractor) and Maricopa County (County), administered by its Human Services Department. The purpose of the Agreement is for the Contractor to provide weather relief accomm odations for individuals experiencing homelessness at the St. Vincent de Paul Watkins Hall of Banners, 420 West Watkins Road, Phoenix, Arizona 85003 location. The purpose of the Amendment is to address the following items: A. Extend the Agreement termina tion date from June 30, 2022, through June 30, 2023. B. Add a Work Statement hereinafter referred to as “Heat Relief Work Statement”, attached and incorporated into the Agreement. The Heat Relief Work Statement is for the Contractor to provide shelter services to people experiencing homelessness who have used the heat relief shelter. The County shall provide Contractor with $2,003,920 for the Heat Relief Work Statement services. Funding is provided by American Rescue Plan Act (ARPA) funds, CFDA No. 21.027. C. The County shall provide Contractor with an additional amount of $88,845 in County General Funds for drinking water for individuals experiencing homelessness over the summer months. All services provided or costs incurred or expensed shall be reimbursable through October 31, 2022. D. The Agreement funding amount shall be increased from $808,503 to $2,901,268. The Agreement is amended to incorporate the changes contained in this Amendment No. 3. All other terms and conditions of the Agreeme nt and previously approved amendments shall remain unchanged and in full force and effect as executed by the Parties. 54. AMENDMENT TO THE DIOCESAN COUNCIL FOR THE SOCIETY OF SAINT VINCENT DE PAUL AGREEMENT FOR TEMPORARY HOMELESS SHELTER Approve Amendment No. 4 to the Agreement between Diocesan Council for the Society of Saint Vincent de Paul (Contractor) and Maricopa County (County), administered by its Human Services Department. The purpose of the Agreement is for the Contractor to provide weather relief accommodations for individuals experiencing homelessness at the St. Vincent de Paul Watkins Hall of Banners, 420 West Watkins Road, Phoenix, Arizona 85003 location. The purpose of the Amendment is to address the following items: A. The County shall provide Contractor with $96,090 in County General Funds for Janitorial Services. The funding availability is through October 31, 2022. The monthly janitorial services are anticipated to be $16,015. B. Add that the County is updating requirements whereby Saint Vincent de Paul and all workers providing services in this agreement will be subject to background and security checks and screening. The Background Screening will comply with all applicable County laws, rules, and regulations. C. The Agreement funding amount shall be increased from $2,901,268 to $2,997,358. The Agreement is amended to incorporate the changes contained in this Amendment No. 4. All other terms and conditions of the Agreement and previously approved amendments shall remain unchanged and in full force and effect as executed by the Parties. AMENDMENT TO THE DIOCESAN COUNCIL FOR THE SOCIETY OF SAINT VINCENT DE PAUL AGREEMENT FOR TEMPORARY HOMELESS SHELTER Approve Amendment No. 5 to the Agreement between Diocesan Council for the Society of Saint Vincent de Paul (Contractor) and Maricopa County (County), administered by its Human Services Department. The purpose of the Agreement is for the Contractor to provide services to individuals experiencing homelessness. The purpose of the Amendment is to address the following items: A. Correct Amendment No. 4 which referenced Amendment No. 3 funding source as Assistance Listing Number (ALN) 81.135 American Rescue Plan Act (ARPA) to correctly reflect the funding source to Coronavirus State And Local Fiscal Recovery Funds ALN 21.027. B. Extend the Agreement term from June 30, 2023, through January 31, 2025. C. Contractor shall continue to provide shelter services to homeless individuals at the St. Vincent de Paul 2739 E. Washington Street location to include meals, laundry and clothing services, support case conferencing efforts and connecting individuals to needed services and housing. D. The County shall provide Contractor with $4,678,948 for services to homeless individuals from November 1, 2022, through January 31, 2025. The Contractor’s monthly expenditure amounts shall not exceed $173,294. Funding for this Amendment No. 5 shall be provided under Coronavirus State And Local Fiscal Recovery Funds ALN 21.027. E. By this Amendment, the total funding amount provided by the County through January 31, 2025, is $7,676,306. The Agreement is amended to incorporate the changes contained in this Amendment No. 5. All other terms and conditions of the Agreement and previously approved amendments shall remain unchanged and in full force and effect as executed by the Parties. The period of funding availability for Amendment No. 7 shall extend through January 31, 2026. The total available funding under this Amendment No. 7 shall not exceed $2,108,421. Contractor shall continue to provide shelter services for homeless individuals at the St. Vincent de Paul 2739 E. Washington Street location to include meals, laundry and clothing services, support case conferencing efforts and connecting individuals to needed services and housing. B. AMENDMENT TO COMPETITION IMPRACTICABLE AGREEMENT WITH DIOCESAN COUNCIL FOR THE SOCIETY OF SAINT VINCENT DE PAUL FOR SHELTER SERVICES TO HOMELESS INDIVIDUALS Approve financial Amendment No. 8 to the Competition Impracticable Agreement between the Diocesan Council for the Society of Saint Vincent de Paul (Contractor) and Maricopa County (County), administered by its Human Services Department. The purpose of the Agreement is for the Contractor to provide services to individuals experiencing homelessness in Maricopa County including providing for basic needs, case management, mental health support, and housing referrals among other services at the East Washington Street Shelter located at 2739 E Washington Street in Phoenix. The Agreement funding amount is $ 9,849,727, provided by the County through ARPA Funds under Assistance Listing Number (“ALN”) 21.027 and County General Funds. The term of the Agreement is June 9, 2021, through January 31, 2026. The purpose of the Amendment is to address the following: A. Extend the Agreement Termination Date from January 31, 2025, through September 30, 2026 B. The County shall provide the Contractor with an additional $1,500,000 in American Rescue Plan Act (“ARPA”) funds under Assistance Listing Number 21.027. The period of funding availability for Amendment No. 8 shall extend through September 30, 2026. Contractor shall continue to provide shelter services for homeless individuals. C. The Agreement funding total amount shall be increased from $9,849,727 to $11,349,727. D. Update lead County point of contact. E. Update required Agreement language.
