41. AMENDMENT WITH ARIZONA DEPARTMENT OF HOUSING FOR THE WEATHERIZATION ASSISTANCE PROGRAM Approve Amendment No. 1 to the revenue Agreement between Arizona Department of Housing (ADOH) and Maricopa County administered by its Human Services Department. The purpose of the Agreement is for ADOH to provide funding to the County to administer the Weatherization Assistance Program. The purpose of Amendment No. 1 is to address the following: A. ADOH Scope of Work has changed to reflect an increase to the LIHEAP average per unit investment limits and revised the unit completion goals. The LIHEAP Weatherization average per unit investment over Program Year 2021 (July 1, 2021 through September 30, 2021) is $10,000 and increases to $20,000 (October 1, 2021 to June 30, 2022). The funding sources are identified in the Original Agreement. LIHEAP - CARES Act funds in the amount of $1,175,012 and ADOH funds in the amount of $624,152. The funding amounts are unchanged. The funds will be utilized to provide Weatherization Assistance Program measures in dwellings occupied by low-income households in Maricopa County outside of the City of Phoenix and the City of Mesa. The revised number of units the County will be required to complete is revised to seventy-four (74) units. Contract expiration remains the same. B. Revise Section 6 (Scope of Work) Attachment A and replace it in its entirety with the attached revised Attachment A. C. Revise Section 7 (Reports) Attachment B and replace it in its entirety with the attached revised Attachment B. The Agreement is amended to incorporate the changes contained in this Amendment No. 1. All other terms and conditions of the Agreement shall remain unchanged and in full force and effect as executed by the Parties. Amendment No. 1 shall be effective upon approval and signature by both Parties. There are no Expenditures associated with Amendment No. 1. Supervisory District: All (C-22-21-127-X-01)
