33. AMENDMENT TO THE IGA WITH THE ARIZONA FIRE AND MEDICAL AUTHORITY COMMUNITY DEVELOPMENT BLOCK GRANT ACTIVITIES Approve Amendment No. 2 to the Intergovernmental Agreement between the Arizona Fire and Medical Authority (“Subrecipient”) and Maricopa County (“County”) administered by its Human Services Department. The purpose of the Amendment is to address the following items: A. Revise Section 3 (Work Statement): i . Paragraph 2.0 (Scope of Work), to remove the Scope of Work in its entirety as addressed in the original Agreement and Amendment No. 1. The new Scope of Work will address the purchasing of a backup generator for the Tonopah, Arizona fire station, to provide electricity during power failures and allow the fire station to provide uninterrupted service to the Tonopah community; Purchase and replace the well water system and parts, which will be used in the fire station for all water-related needs; and Install security perimeter fencing around the fire station to secure its resources. ii. Paragraph 3.0 (Implementation Schedule), remove the Implementation Schedule in the original Agreement in its entirety and replace with a new Implementation Schedule. B. Revise Section 4 (Budget and Compensation). The funding for this Agreement remains at $214,748. The anticipated funding for FY22 is $100,000, and FY23 is $114,748. C. Funding for Amendment No. 2 is provided by U.S. Department of Housing and Urban Development (HUD) Community Development Block Grant (CDBG) funds. The Agreement is amended to incorporate the changes contained in this Amendment No. 2. All other terms and conditions of the Agreement shall remain unchanged and in full force and effect as executed by the Parties. Amendment No. 2 shall be effective upon approval and signature by both Parties. Expenditures: $0. Supervisory District 5 (C-22-21-132-X-02)

AMENDMENT TO IGA WITH THE ARIZONA FIRE AND MEDICAL AUTHORITY COMMUNITY DEVELOPMENT BLOCK GRANT ACTIVITIES Approve Amendment No. 3 to the Intergovernmental Agreement between the Arizona Fire and Medical Authority (“Subrecipient”) and Maricopa County (“County”) administered by its Human Services Department. The purpose of this Agreement is for Subrecipient to install emergency equipment in case of power outages in the Tonopah area. The County has provided the Subrecipient with $214,748 in U.S. Department of Housing and Urban Development (HUD) Community Development Block Grant (CDBG) funds. The purpose of the Amendment is to address the following items: A. Extend the Termination Date from June 30, 2023, through June 30, 2024. B. Revise Section 3 (Work Statement) Paragraph 3.0 (Implementation Schedule), remove the Implementation Schedule in the original Agreement in its entirety and replace with a new Implementation Schedule. The Agreement is amended to incorporate the changes contained in this Amendment No. 3. All other terms and conditions of the Agreement and previously approved amendments shall remain unchanged and in full force and effect as executed by the Parties. Amendment No. 3 shall be effective upon approval and signature by both Parties. This Amendment does not have financial impact. Supervisory District 5. (C-22-21-132-X-03)

The purpose of this Agreement is for Subrecipient to install emergency equipment in case of power outages in the Tonopah and Wittmann areas. The County has provided the Subrecipient with $766,648 in U.S. Department of Housing and Urban Development (HUD) Community Development Block Grant CARES Act (CDBG-CV) funds. The purpose of the Amendment is to address the following items: A. Extend the Termination Date from June 30, 2024, through October 31, 2025. B. Revise Section 3 (Work Statement), Paragraph 3.0 (Implementation Schedule) to delete in its entirely and replace with a new implementation schedule. The term of the Agreement is July 1, 2021, through October 31, 2025. The purpose of the Amendment is to address the following items: A. Extend the Termination Date from October 31, 2025, through August 31, 2026. B. In Section 1 (General Provisions), revise and replace Paragraph 12 (Budget Adjustments) C. In Section 2 (Special Provisions), Paragraph 7.0 (Administrative requirements) revise and replace 1. Subparagraphs 7.4.2 SAM and 7.7.2 UEI Number 2. Paragraphs 13.3.18 (Administrative Change Orders) 3. and Add Paragraph 14 (Reversion of Assets) to the Agreement D. In Section 3 (Work Statement) revise and replace Paragraph 3.0 (Implementation Schedule) Amendment No. 6 shall be effective upon approval and signature by both Parties. Supervisory District 4.
