119. IGA WITH TOWN OF GILA BEND Approve an Intergovernmental Agreement (“Agreement”) between the Town of Gila Bend (“Subrecipient”) and Maricopa County (the “County”) administered by its Human Services Department. The purpose of the Agreement is to address the following: 1. Subrecipient to complete two project activities: A. Scope of Work A – To renovate the Gila Bend Community Center (“Center”), located at 202 East Euclid Avenue, Gila Bend, AZ 85337, to expand the client service and delivery area. The renovations include the construction of an 800 square foot addition and reconfiguration of office spaces. The changes to the Center will improve the entrance into the building and allow for social distancing, reducing the risk of spreading COVID-19. Period of funding availability is December 1, 2021 through January 31, 2023. The County will provide the Subrecipient with U.S. Department of Housing and Urban Development (HUD) Community Development Block Grant CARES Act (CDBG-CV) funds in the amount not-to-exceed $450,000. B. Scope of Work B -To reconstruct approximately two hundred and fifty (250) feet of existing eight (8) inch sewer lines at two (2) locations along the south side of Papago Street between Washington Street and Cholla Avenue, and to restore existing sewer service that was damaged during the August 2021 flood. The sewer line rehabilitation project will restore the existing sewer service system to pre-flood conditions to ensure continual service to the residents of Gila Bend. Period of funding availability is December 1, 2021 through March 31, 2023. The County will provide the Subrecipient with American Rescue Plan Act (ARPA) funds in the amount not-to-exceed $500,000. 2. The County shall provide the Subrecipient with a total of $950,000 for the project activities. 3. Agreement term shall begin December 1, 2021 through March 31, 2023. 4. Per Section 2 (Special Provisions), Paragraph 13.0 (Additional Certifications and Warranties), Subparagraph 13.1.18, also request authorization for the Chairman of the Board of Supervisors to approve and execute upon the recommendation of the Human Services Department Director and Legal Counsel to make changes within the general scope of the Agreement on behalf of the County through Administrative Change Orders. Administrative Change Orders shall be approved and fully executed by the Chairman of the Board of Supervisors and the authorized representative for the Subrecipient. Change Orders/Administrative Changes will receive legal review and will be filed with the Clerk of the Board’s office upon full execution. FY2022: $950,000 Supervisory Districts 5 (C-22-22-050-X-00)

38. AMENDMENT TO IGA WITH TOWN OF GILA BEND Approve financial Amendment No. 1 to an Intergovernmental Agreement (“Agreement”) between the Town of Gila Bend (“Subrecipient”) and Maricopa County (the “County”) administered by its Human Services Department. The County and the Subrecipient are referred to collectively as the “Parties.” The purpose of the Agreement is for the Subrecipient to renovate the Gila Bend Community Center and reconstruct approximately two hundred and fifty (250) feet of existing eight (8) inch sewer lines. The County has provided the Subrecipient with $450,000 i n U.S. Department of Housing and Urban Development (HUD) Community Development Block Grant CARES Act (CDBG-CV) funds under Assistance Listing Number (ALN) 14.218. With this Amendment, The County shall provide Subrecipient with $50,000 in U.S. Department of Housing and Urban Development (HUD) Community Development Block Grant CARES Act (CDBG-CV) funds under Assistance Listing Number (ALN) 14.218. The purpose of this Amendment No. 1 is to address the following: A. The County shall provide the Subrecipient with an additional $50,000 to include reconfiguration of the Community Center HVAC system to help reduce the potential spread of infectious viruses through the HVAC system. B. The Agreement amount shall now be increased to a total not-to-exce ed amount of $1,000,000. C. Revise Section 2 (Special Provisions), Paragraph 4.0 (Audit Requirements) to address the Audit Requirements process. D. Revise Section 3 (Work Statement), Paragraph 2.0 (Scope of Work) to delete and replace Subparagraph 2.1 (P roject Description). E. Revise Section 4 (Budget and Compensation), Paragraph 1.0 (Budget). The Agreement is amended to incorporate the changes contained in this Amendment No. 1. All other terms and conditions of the Agreement shall remain unchanged and in full force and effect as executed by the Parties. 34. The purpose of the Agreement is for the Town to renovate the Gila Bend Community Center and to reconstruct sewer lines in the town. The purpose of this Amendment No. 4 is to address the following: A. Change Lead Agency Point of Contact. B. Extend the Agreement term from September 30, 2023, through September 30, 2024. C. Revise Section 3 (Work Statement) in the following areas: 2.0 Scope of Work A, 2.6 (Implementation Schedule) to extend the activity timelines. 3.0 Revise Scope of Work B, 3.6 Period of funding from July 26, 2023 through September 30, 2024. The Agreement is amended to incorporate the changes contained in this Amendment No. 4. All other terms and conditions of the Agreement and previously approved amendments shall remain unchanged and in full force and effect as executed by the Parties. 50. AMENDMENT TO THE IGA WITH TOWN OF GILA BEND FOR COMMUNITY DEVELOPMENT BLOCK GRANT ACTIVITIES Approve non-financial Amendment No. 6 to an Intergovernmental Agreement (IGA) between the Town of Gila Bend and Maricopa County administered by its Human Services Department. The purpose of the IGA is for the Town to renovate the Gila Bend Community Center located at 202 East Euclid Avenue, Gila Bend, AZ 85337 and to reconstruct sewer lines and fully restore sewer service in the Town that were destroyed by a flood, outlined through two separate Scopes of Work. The County has provided the Subrecipient with U.S. Department of Housing and Urban Development (HUD) Community Development Block Grant CARES Act (CDBG-CV) funding of $1,000,000 under Assistance Listing Number (ALN) 14.218 and American Rescue Plan Act (ARPA) - State and Local Fiscal Recovery Funds (SLFRF) in the amount of $500,000 under ALN 21.207. The County has provided the Subrecipient with a total of $1,500,000 for the work statement activities identified. The purpose of this Amendment No. 6 is to address the following: A. Extend the Agreement term through September 30, 2025. B. Incorporate Federal Award Identification Number (FAIN) and Award Date under ALN 21.027 by adding to Page 1 of the IGA. C. Section 1 (General Provisions) Revise Paragraph 3.0 (Renewal) and replace it in its entirety with required language. D. Revise Section 3 (Work Statement) to update Paragraph 2.0 (Scope of Work) and replace Implementation Schedule in Subparagraph 2.6 in its entirety with an updated project timeline. The Agreement is amended to incorporate the changes contained in this Amendment No. 6. All other terms and conditions of the Agreement and previously approved amendments shall remain unchanged and in full force and effect as executed by the Parties.
