139. SOLE SOURCE AGREEMENT WITH HUMAN SERVICES CAMPUS, INC. FOR SHELTER SERVICES Approve a Sole Source Agreement (“Agreement”) between Human Services Campus, Inc, a non-profit, community-based organization (“Subrecipient”) and Maric opa County administered by its Human Services Department. The purpose of the Agreement is for the Subrecipient to provide services to individuals/clients experiencing homelessness. The County will provide the Subrecipient with $2,799,487 to conduct five homelessness project activities: 1. Street to Home Coordinator; 2. Hotel Shelter Case Management Operations; 3. Weather Relief and Overflow Shelters; 4. Emergency Shelter 5. Administration and Compliance Funding for the Agreement is provided under the following fund sources Assistance Listing Numbers 21.027, American Rescue Plan Act, Coronavirus State and Local Fiscal Recovery Funds and Maricopa County General funds allocated to the Human Services Department. The Agreement term is July 1, 2022 thro ugh June 30, 2024 and will be effective upon approval and signature by both Parties. This Agreement is subject to the provisions of A.R.S. §38-511. Also request authorization for the Chairman of the Board of Supervisors to approve and execute Administrati ve Change Orders upon the recommendation of the Human Services Director and Legal Counsel as identified in Paragraph 6.0 (Administrative Change Orders). 53. AMENDMENT TO THE AGREEMENT WITH HUMAN SERVICES CAMPUS, INC. FOR SHELTER SERVICES Approve Amendment No. 1 to the Agreement between Maricopa County (“County”), administered by its Human Services Department, and Human Services Campus, Inc., (“Subrecipient”). The County and the Subrecipient are referred to collectively as the “Parties.” The purpose of the Agreement is for the Subrecipient to provide services to individuals/clients experiencing homelessness. The purpose of the Amendment is to address the following items: A. Correct an administrative error that incorrectly referenced Human Services Campus, LLC. The Subrecipient should be referred to as Human Services Campus, Inc. B. Remove Attachment A (Projects Scope and Requirements) in the original Agreement and replace with a revised Attachment A (Projects Scope and Requires). The Agreement is amended to incorporate the changes contained in this Amendment No. 1. All other terms and conditions of the Agreement shall remain unchanged and in full force and effect as executed by the Parties. 27. AMENDMENT TO AGREEMENT WITH HUMAN SERVICES CAMPUS, INC. FOR SHELTER SERVICES Approve Amendment No. 2 to the Agreement between Maricopa County (“County”), administered by its Human Services Department, and Human Services Campus, Inc., (“Subrecipient”). The County and the Subrecipient are referred to collectively as the “Parties.” The purpose of the Agreement is for the Subrecipient to provide services to individuals/clients experiencing homelessness. The purpose of the Amendment is to address the following items: A. Revise paragraph 3.0 (Funding, Invoicing and Payments) in the following sub-subparagraphs which address the following: 3.1.1 Reduce Street to Home Coordinator by $35,000 from $69,056 to a new amount of $34,056; 3.1.3 Increa se Weather Relief and Overflow Shelters line item by $65,000 for a new line-item total of $1,443,325 by: • Reallocate $35,000 from line 3.1.1; and. • The County shall provide the Subrecipient with $30,000 in County General funds. The expenditure period fo r these funds is through June 30, 2023. B. The Agreement funding amount shall be increased from $2,799,487 to $2,829,487. C. Revise the Agreement to add the following new paragraph: 50.0 UYGHUR FORCED LABOR PREVENTION ACT (UFLPA) The Agreement is amende d to incorporate the changes contained in this Amendment No. 2. All other terms and conditions of the Agreement shall remain unchanged and in full force and effect as executed by the Parties. The County and the Subrecipient are referred to collectively as the “Parties.” The purpose of the Agreement is for the Subrecipient to provide services to individuals/clients experiencing homelessness. The purpose of the Amendment is to address the following items: A. Revise paragraph 3.0 (Funding, Invoicing and Payments) which addresses the following: 1. Reduce Street to Home Coordinator by $18,056 from $34,056 to a new amount of $16,000; The expenditure period for these funds will expire on June 30, 2023. 2. Increase Hotel Shelter Case Management Operations line item by $692,578 from $692,106 for a new line-item total of $1,384,684. 3. Increase Weather Relief and Overflow Shelters line item by $1,440,023 from $1,443,325 for a new line-item total of $2,883,348. 4. Increase Emergency Shelter by $600,000 from $600,000 to a new line-item total of $1,200,000. 5. Increase Administration and Compliance by $60,000 from $60,000 for a new line-item total of $120,000. 6. Funding for the Amendment is provided under the following fund sources: a. The County shall provide the Subrecipient with $2,132,601 in ARPA funds (ALN) 21.027. b. The County shall provide the Subrecipient with $660,000 in County General funds. The expenditure period for these funds is through June 30, 2024. 7. Amendment No. 3 funding amount is $2,774,545 (an increase of $2,792,601 and a decrease of $18,056). 8. With this Amendment, the Agreement funding total is $5,604,032. a. $1,382,106 in County General Funds b. $4,239,982 in ARPA funds The Agreement is amended to incorporate the changes contained in this Amendment No. 3. All other terms and conditions of the Agreement shall remain unchanged and in full force and effect as executed by the Parties. The revised Emergency Shelter activities shall be effective as of July 1, 2023. The Agreement is amended to incorporate the changes contained in this Amendment No. 4. 62. AMENDMENT TO AGREEMENT WITH HUMAN SERVICES CAMPUS, INC. FOR SHELTER SERVICES Approve financial Amendment No. 6 to the Agreement between Human Services Campus, Inc., (“Subrecipient”) and Maricopa County (“County”), administered by its Human Services Department. The purpose of the Agreement is for the Subrecipient to provide services to individuals/clients experiencing homelessness. The Agreement term began July 1, 2022, through February 28, 2025. The Agreement total funding is $ 5,875,520. The purpose of the Amendment is to address the following items: 1. Increase Agreement funding by $350,000 ARPA funds under ALN 21.207. 2. Extend the termination date of the Agreement from February 28, 2025, through March 31, 2025 3. Revise Paragraph 3.0 (Funding, Invoicing and Payments) to address funding line items and expenditure timeframe. 4. The Agreement funding is increased to a new total not-to-exceed amount of $6,225,520. 5. Update required Agreement language 6.
