52. AMENDMENT TO AGREEMENT WITH HOM, INC. FOR A CENTRALIZED, SYSTEMWIDE LANDLORD ENGAGEMENT SERVICE Approve financial Amendment No. 1 to the Agreement between HOM, Inc. (“Contractor”) and Maricopa County (the “County”) administered by its Human Services Department. The purpose of the Agreement is for the Contractor to provide a centralized, systemwide landlord engagement service for the homeless population in Maricopa County. The purpose of the Amendment No. 1 is to address the following: A. Extend the Contract term from June 30, 2023, through June 30, 2025. B. Increase funding amount by $2,500,000 from $4,970,556 to a not to exceed amount of $7,470,556. C. Revise Section 1.0 (General Provisions) by adding the following paragraphs: 52.0 UYGHUR FORCED LABOR PREVENTION ACT (UFLPA) 53.0 FORCE MAJEURE D. Revise Section 2 (Special Provisions) in the following subparagraphs (Documentation and Recording Keeping) and (Subcontracts) by removing in its entirely and replacing with revised contract language. E. Revise Section 3.0 (Work Statement) subparagraph 3.0 (Performance Management Deliverables), sub-subparagraph 3.7 by removing in its entirely and replacing with revised contract language. F. Revise Section 4 (Budget and Compensation) by addressing the following: 1.0 Remove the Budget in its entirety and replace with a revised budget. The Agreement is amended to incorporate the changes contained in Amendment No. 1. All other terms and conditions of the Agreement shall remain the same and unchanged and in full force and effect as executed by the Parties. This Amendment No. 1 shall be effective upon approval and signature by both parties. Supervisory District: All (C-22-22-151-X-01)

49. AMENDMENT TO COMPETITION IMPRACTICABLE AGREEMENT WITH HOM, INC. Approve financial Amendment No. 3 to the Competition Impracticable Agreement between HOM, Inc. and Maricopa County administered by its Human Services Department. The purpose of the Agreement is for the Contractor to provide a centralized, system wide landlord engagement service for the homeless population in Maricopa County. The term of the Agreement is through June 30, 2025. The purpose of the Amendment No. 3 is to address the following: A. The County shall increase the funding amount by $29,444 in American Rescue Plan Act (ARPA) funds from $7,470,556 for a new total revised budget amount not-to-exceed $7,500,000. B. Revise Contract language. This Amendment No. 3 shall be effective upon approval and signature by both parties. Supervisory District: All (C-22-22-151-X-03)

92. AMENDMENT TO COMPETITION IMPRACTICABLE AGREEMENT WITH HOM, INC. Approve financial Amendment No. 4 to the Competition Impracticable Agreement (Agreement) between HOM, Inc. (Contractor) and Maricopa County (County) administered by its Human Services Department. The purpose of the Agreement is for the Contractor to provide a centralized, systemwide landlord engagement service for the homeless population in Maricopa County. The County provided the Contractor with $7,500,000 in American Rescue Plan Act (ARPA) State and Local Fiscal Recovery Funds (SLFRF) under Assistance Listing Number 21.027, provided to the County by the US Department of Treasury. The term of the Agreement is July 14, 2021, through June 30, 2025. The purpose of the Amendment No. 4 is to address the following: A. Update the County Point of Contact B. Extend the Agreement term through December 31, 2026. C. The County shall increase the funding amount by $600,000 in County General Funds for a new Agreement total funding amount of $8,100,000. D. Revise Contract language. E. Revise and replace with a new Agreement budget. This Amendment No. 4 shall be effective upon approval and signature by both parties. (Contractor) and Maricopa County (County) administered by its Human Services Department. The purpose of the Agreement is for the Contractor to provide a centralized, systemwide landlord engagement service for the homeless population in Maricopa County. The County provided the Contractor with $7,500,000. in American Rescue Plan Act (ARPA) State and Local Fiscal Recovery Funds (SLFRF) under Assistance Listing Number 21.027, provided to the County by the US Department of Treasury, and $600,000 in County General funds were provided, for an Agreement total funding amount of $8,100,000. The current term of the Agreement is July 14, 2021, through December 31, 2026. The purpose of Amendment No. 5 is to address the following: A. Update the County Point of Contact B. The County shall increase the funding amount by $100,000 in County General Funds, funds to be fully expended by 6/30/2026 C. The Agreement funding shall hereby increase from $8,100,000 to a new total agreement amount of $8,200,000 D. Revise Contract language in Section 1 General Provisions E.
