66. ASSET DISPOSITION AND DONATION OF HEAD START PROGRAM IMPROVEMENT ITEMS TO GREAT EXPLORES DBA DESERT FRIENDS LLC Request authorization to dispose of and donate facility improvements made to the Great Explores DBA: Desert Friends LLC, an Arizona corporation location at 15637 N. Hollyhock Street, Surprise, AZ. The improvements were purchased by Maricopa County Head Start Program in June 2016 utilizing grant funds from U.S. Department of Health and Human Services/Administration for Children and Families/Office of Head Start. Improvements to the location were required to ensure Head Start performance standards were met. Great Explores DBA: Desert Friends LLC contracted with the County as a Head Start Childcare provider from June 2015 through November 30, 2021. The Human Services Department requests disposition and donation of the facility improvements to Great Explores DBA: Desert Friends LLC. Approval of this action will result in a net decrease in the Human Services Department Asset Inventory list. Facility improvements include: •Infant room: flooring, countertops and sinks, storage cabinets •Roof repairs The Human Services Department submitted a request to the Office of Head Start through the TANGIBLE PERSONAL PROPERTY REPORT Disposition Request/Report SF-428-C and received approval on October 15, 2024, to dispose of the listed property.
