48. AMENDMENT TO LEASE AGREEMENT WITH FIRST PRESBYTERIAN CHURCH FOR HEAD START/EARLY HEAD START PROGRAM Approve and execute Amendment No. 7 to Lease Agreement No. L-7271, with First Presbyterian Church, located at 161 North Mesa Drive, Mesa, AZ, effective January 1, 2010. The purpose of the amendment is to extend the total term of the lease by two months, through September 30, 2011 and to adjust the annual operating expenses to account for the two additional months. The amendment also provides for additional leased space in the form of the Church Nursery to be used as an Early Head Start classroom, and an area on the existing playground for use as an Early Head Start playground. The rental fee for the additional Early Head Start classroom space is $923.00 per month for the period of January 1, 2010 through September 30, 2011 or $19,383 total for the 21 month lease period for this additional classroom. This rental fee is funded through the Early Head Start American Recovery and Reinvestment Act (ARRA) Expansion Funds and does not impact any County General funds. The rental fee for the existing Head Start classroom will remain at $660.66 per month for the original term of the contract and for the additional two months of August and September, 2011. The Early Head Start Program will use the additional Early Head Start leased space for the purpose of providing a comprehensive developmental preschool experience for eight children, ages 1-3 years. 59. AMENDMENT TO LEASE AGREEMENT WITH FIRST PRESBYTERIAN CHURCH FOR HEAD START/EARLY HEAD START PROGRAM Approve and execute Amendment No. 8 to Lease Agreement No. L-7271, with First Presbyterian Church, located at 161 North Mesa Drive, Mesa, AZ. The purpose of the amendment is to extend the total term of the lease by three years, effective October 1, 2011 through September 30, 2014. The rental fee for the Head Start/Early Head Start classroom space remains at $1,583.66 per month for the period of October 1, 2011 through September 30, 2014 or $57,011.76 total for the 36 month lease period. There is no increase to the current rate. This rental fee is funded through the Head Start/Early Head Start base grant and does not impact any County General funds. The Early Head Start Program uses the Early Head Start leased space for the purpose of providing a comprehensive developmental preschool experience for eight children, ages 1-3 years. 28. OPTION TO RENEW LEASE AGREEMENT WITH FIRST PRESBYTERIAN CHURCH OF MESA Approve and authorize the Chairman to execute Receipt of Option to Renew Notification, L-7271 Amendment No. 9 with First Presbyterian Church of Mesa, an Arizona non-profit religious corporation for use of space at 161 N. Mesa Drive, Mesa, AZ. The County is exercising its option to renew for three (3) years commencing October 1, 2014 and expiring September 30, 2017. The rental fee for the Head Start/Early Head Start classroom space remains at $1,584.32 plus rental tax per month. The rental fee is funded through the Head Start/Early Head Start base grant and does not impact any County general funds. Either Party may terminate the Lease with a 60-day written notice to the other. (C-22-99-134-4-09)

48. AMENDMENT TO LEASE AGREEMENT WITH FIRST PRESBYTERIAN CHURCH OF MESA Approve and execute Amendment No. 14 to Lease Agreement L-7271 with First Presbyterian Church of Mesa, for the use of approximately 1,583 square feet of classroom space located at 161 North Mesa Drive, Mesa, Arizona to provide Head Start Services. This purpose of the amendment is to address the following: (A) ratify approval of certain Lessee Improvements to the existing sandbox will be replaced with 585 sf of turf on the East side and 600 sf of concrete on the West side, when this project is complete there will no longer be a sandbox at a one-time cost of up to $45,000; (B) amend alterations provisions; (C) amend Notices; and (D) provide for lease administration authority. The Agreement is subject to termination pursuant to the provisions of A.R.S. 38-511. Either Lessor or Lessee may terminate the Lease by giving not less than sixty (60) days prior written notice of termination to the other Party. All other terms and conditions remain the same. Improvements to the Leased space will be paid through the U.S. Department of Health and Human Services (DHHS)/Administration for Children and Families (ACF), Office of Head Start (OHS), Grant No. 09CH010385.
