35. BUSINESS ASSOCIATE AGREEMENT WITH MARICOPA MERCY INTEGRATED CARE Approve an Agreement between Mercy Maricopa Integrated Care and Maricopa County through the Department of Correctional Health Services, for the purpose of entering into a Business Associate Agreement (BAA) per Health Insurance Portability and Accountability Act (HIPPA). This Agreement is non-financial and is effective from February 12, 2014 until February 11, 2017. Enter into a BAA with Mercy Maricopa Integrated Care, the new Maricopa County Regional Behavioral Health Care Agency. (C-26-14-003-3-00)

23. AMENDMENT TO BUSINESS ASSOCIATE AGREEMENT WITH MERCY MARICOPA INTEGRATED CARE Approve Amendment No. 1 to an Agreement between Mercy Maricopa Integrated Care and Maricopa County through the Department of Correctional Health Services. The purpose of this amendment is to include secure web portal registration form and update business associate agreement language. This amendment is non-financial and is effective from 4/8/2014 until 2/11/2017. All other terms and condistions of the Agreement remain in full force and effect. (C-26-14-003-3-01)
