48. MOU WITH BLUE CROSS BLUE SHIELD ARIZONA HEALTH CHOICE FOR CARE COORDINATION Approve the Memorandum of Understanding (MOU) between Blue Cross Blue Shield Arizona Health Choice (BCBSAZ Health Choice) and Maricopa County by and through its Department of Correctional Health Services (CHS). This MOU establishes a collaborative protocol for effective communication, coordination, and continuity of care for individuals eligible for services provided by BCBSAZ Health Choice transitioning from incarceration to the community. Specifically, BCBSAZ Health Choice provides “reach-in” care coordination for members who have been incarcerated in the Maricopa County Jails for 20 days or longer and have an anticipated release date. “Reach-in” care coordination activities shall begin upon knowledge of a member’s anticipated release date. BCBSAZ Health Choice will collaborate with CHS to identify justice-involved members in the adult criminal justice system with physical and/or behavioral health chronic and/or complex care needs prior to member’s release. There are no financial responsibilities assigned to either party by this MOU. (C-26-25-003-X-00)

70. AMENDMENT TO MOU WITH BLUE CROSS BLUE SHIELD ARIZONA HEALTH CHOICE Approve Amendment No. 1 for the Memorandum of Understanding (MOU) between Blue Cross Blue Shield Arizona Health Choice (BCBSAZ Health Choice) and Maricopa County by and through its Department of Correctional Health Services (CHS). This MOU aims to establish a collaborative protocol for effective communication, coordination, and continuity of care for individuals eligible for services provided by BCBSAZ Health Choice who are also served by CHS. The purpose of this amendment is to renew for a term of twenty-four (24) months. This non-financial amendment is effective November 13, 2025, and ends on November 13, 2027. Additionally, all references to “Blue Cross Blue Shield Arizona Health Choice” and “Health Choice AZ Inc.” will be removed and replaced with “Health Choice Arizona, Inc.” to reflect the updated naming convention. All other terms and conditions of the MOU will remain in effect. (C-26-25-003-X-01)
