50. MOU WITH BANNER UNIVERSITY HEALTH PLANS FOR CARE COORDINATION Approve the Memorandum of Understanding (MOU) between Banner University Health Plans (BUHP) and Maricopa County by and through its Department of Correctional Health Services (CHS). This MOU establishes a collaborative protocol for effective communication, coordination, and continuity of care for individuals eligible for services provided by BUHP transitioning from incarceration to the community. Specifically, BUHP provides “reach-in” care coordination for members who have been incarcerated in the Maricopa County Jails for 20 days or longer and have an anticipated release date. “Reach-in” care coordination activities shall begin upon knowledge of a member’s anticipated release date. BUHP will collaborate with CHS to identify justice-involved members in the adult criminal justice system with physical and/or behavioral health chronic and/or complex care needs prior to member’s release. There are no financial responsibilities assigned to either party by this MOU. (C-26-25-005-X-00)

71. AMENDMENT TO MOU WITH BANNER UNIVERSITY HEALTH PLANS Approve Amendment No. 1 for the Memorandum of Understanding (MOU) between Banner University Health Plans (BHUP) and Maricopa County by and through its Department of Correctional Health Services (CHS). This MOU aims to establish a collaborative protocol for effective communication, coordination, and continuity of care for individuals eligible for services provided by BHUP who are also served by CHS. The purpose of this amendment is to renew for a term of twenty-four (24) months. This non-financial amendment is effective November 7, 2025, and ends on November 7, 2027. Additionally, the primary point of contact for Sections IV-Q-5 and IV-R-6 will be updated. All other terms and conditions of the MOU will remain in effect. (C-26-25-005-X-01)
