JOSEPH'S HOSPITAL AND MEDICAL CENTER SERVICE AGREEMENT Approve Amendment No. 1 to a Service Agreement between Catholic Healthcare West d/b/a St. Joseph's Hospital and Medical Center and Maricopa County through the Office of Medical Examiner in the not-to-exceed amount of $3,000. The purpose of this amendment is to renew terms of the agreement. This amendment is effective from May 1, 2010 to January 31, 2011. The purpose of this amendment is to renew terms. With the current agreement expiring on January 31, 2015, Dignity Health has asked to amend the current agreement. This amendment is effective from February 1, 2015 to January 31, 2017. All other terms and conditions of the Service Agreement remain in full force and effect. (C-29-08-005-0-05)

29. ADMINISTRATIVE CORRECTION TO TEACHING AGREEMENT WITH DIGNITY HEALTH Approve an Administrative Correction to Fourth Amendment to the Teaching Agreement between Dignity Health d/b/a St. Joseph's Hospital and Medical Center and Maricopa County through the Office of Medical Examiner approved on February 4, 2015. The purpose of this administrative correction is to delete the current Section 12, Insurance, and replace it with a new Section 12, Insurance, and add the Master List. This amendment is effective from February 1, 2015 to January 31, 2017. All other terms and conditions of the Affiliation Agreement remain in full force and effect. (C-29-08-005-0-06)
