CLINICAL AFFILIATION AGREEMENT WITH MIDWESTERN UNIVERSITY Approve an Affiliation Agreement between Midwestern University and Maricopa County through the Office of Medical Examiner, for the purpose of providing a coordinated clinical educational program for the education and training of medical students. This Affiliation Agreement is non-financial and is effective from December 1, 2017. The initial term of this Agreement shall be for five (5) years from the Effective Date. Thereafter, this Agreement may be extended up to three (3) additional successive one (1) year periods (each a “Renewal Term”), unless terminated as provided herein. (C-29-18-004-3-00)
