30. DENTAL INSURANCE PROGRAM RATES AND RECOMMENDATIONS FY11-12 1. Approve full and part-time employee and employer dental benefit rates for the fully insured Delta Dental Plan effective July 1, 2011 through June 30, 2012 for active employees. (On file in the Clerk of the Board's office.) 2. Approve full and part-time employee and employer dental benefit rates for the self-insured CIGNA Dental Plan effective July 1, 2011 through June 30, 2012 for active employees. (On file in the Clerk of the Board's office.) 3. Approve full and part-time employee and employer dental benefit rates for the fully insured Employers Dental Services Dental Plan effective July 1, 2011 through June 30, 2012 for active employees. (On file in the Clerk of the Board's office.) 4. Approve the dental benefit rates for retirees effective July 1, 2011 through June 30, 2012. (On file in the Clerk of the Board's office.) 5. Approve the plan design changes for the Delta Dental Plan as listed in the Additional Information. The Rate Sheet illustrates the monthly Premium (Cost) and employer cost for the dental benefit offerings effective July 1, 2011 through June 30, 2012 for full-time and part-time employees and retirees.
