35. VOLUNTARY INSURANCE PROGRAM PLAN CHANGES AND RATES FOR FY 2018-19 1. Approve continuation of the Employee Flexible Spending Account Plan for the plan year July 1, 2018 through June 30, 2019 with plan changes outlined in the attached Exhibit A. 2. Approve the Maricopa County Employee Short-Term Disability Benefit rates for active employees as outlined in the attached Exhibit B. The attached Exhibit A describes plan changes to Flexible Spending Account to be effective July 1, 2018. The attached Exhibit B illustrates the monthly total employee cost to support the Short-Term Disability Plan for active employees for FY 2018-19. Exhibit C is the Employee Flexible Spending Account Plan Document that was approved by the Board of Supervisors for FY 2017-18.
