36. WELLNESS INCENTIVE PLAN FY2019-2020 Approve the Wellness Incentive Plan for FY2019-2020 as described in the attached Exhibit A. The Wellness Incentive Program will support employees and provide them with activities that assist with improving their health and well-being. Maricopa County cares about the health and well-being of its employees and is committed to reducing healthcare costs and illness-related employee absences by continuing to provide them with an incentive program based on the latest evidence-based, best practices. Eligibility: Employees who are enrolled in a County-sponsored medical benefit plan Incentive: $60 per month Requirements: To earn the FY2019-2020 incentive, eligible employees must complete three activities by June 30, 2019. * Online Health Assessment * Annual Preventive Exam and “My Numbers” Attestation * “Your Choice” Activity (See attached Exhibit A) Other wellness program enhancements: * Pilot the “Ignite” program - a 12-week program for high-risk participants addressing obesity and metabolic syndrome. * Offer a $25 subsidy for a Fit Bit device * Contract with multiple vendors to provide food and nutrition workshops, demonstrations, cooking events and healthier food for groups and individuals.
