AMEND WELLNESS INCENTIVE PLAN FY 21-22 Approve modification of the FY 21-22 Wellness Incentive Program activities to allow employees to earn the premium reduction incentive by completing the Online Health Check Survey and two of the “Your Choice” activities in the Wellness Portal. Due to the COVID-19/coronavirus pandemic, many Wellness Incentive Program activities are no longer feasible. This amendment will modify activities for earning the premium reduction incentive from: *Online Health Assessment *Annual Preventive Exam and “My Numbers” Attestation *“Your Choice” Activity To: *Online Health Check Survey (formerly called Health Assessment) *Two “Your Choice” Activities The total available incentive remains unchanged at $60.00 per month, per employee.
