49. VOLUNTARY BENEFITS RATES FOR FY 21-22 1. Approve continuation of the Flexible Spending Program for the plan year July 1, 2021 through June 30, 2022. Approve any change in the annual contribution limit for 2021 as released by the IRS. 2. Approve the Short-Term Disability Benefit rates for plan year July 1, 2021 through June 30, 2022 as outlined on the attached Exhibit A. 3. Approve continuation of the Employee Assistance Program (EAP) for the plan year July 1, 2021 through June 30, 2022 including rates as reflected on the attached Exhibit A. 4. Approve continuation of County-sponsored basic life and AD&D insurance benefits for the plan year July 1, 2021 through June 30, 2022 including rates as reflected on the attached Exhibit A. 5. Approve continuation of offering additional life and AD&D insurance at the employees’ sole expense for the plan year July 1, 2021 through June 30, 2022 including rates as reflected on the attached Exhibit A. The attached Exhibit A illustrates the monthly total employee cost for the benefit rates for active employees. Exhibit B includes The Maricopa County Benefits Plan (Eleventh Amendment and Restatement) that was approved by the Board of Supervisors for FY 20-21 and which governs how employee health and welfare benefits (such as medical, prescription, behavioral health, dental, flexible spending accounts, etc.) are administered.
