47. VISION BENEFIT RATES FOR FY 21-22 1. Approve the Vision Benefit rates for active employees for July 1, 2021 through June 30, 2022 as outlined in the attached Exhibit A. The attached Exhibit A illustrates the monthly total cost, employee cost, and employer cost for the vision benefit for FY 21-22. Exhibit B includes The Maricopa County Benefits Plan (Eleventh Amendment and Restatement) that was approved by the Board of Supervisors for FY 20-21 and which governs how employee health and welfare benefits (such as medical, prescription, behavioral health, dental, flexible spending accounts, etc.) are administered.
