40. COVID 19 VACCINE INCENTIVE EXTENSION Extend the COVID 19 vaccine incentive that was approved by the Board on 9/1/21 (C-31-22-118-X-00) to 11/30/21. Employees must submit proof of being fully vaccinated to Human Resources by close of business 12/8/21. For employees who submit proof of being fully vaccinated by the original deadline of 11/10/21, it is anticipated that the $100 incentive and credit for the recognition leave will appear on the 11/24/21 paycheck. For employees who submit their proof by 12/8/21, it is anticipated that the $100 incentive and credit for the recognition leave will appear on the 12/22/21 paycheck. Recognition Leave given under this incentive will not count towards a department’s 10% cap granted under the Employee Leave Policy (HR2415). This Recognition Leave must be used by 12/31/2022. The County Manager or designee may approve administrative exceptions to this incentive that are consistent with the intent of the incentive program. This incentive applies to the County’s elected offices, appointed departments, and the Flood Control District of Maricopa County and the Maricopa County Library District (Special Districts). The Board of Supervisors is authorized to jointly adopt policies applying to the Special Districts under the Intergovernmental Agreement, C-06-18-393-6-00, approved on April 11, 2018.
