40. COVID 19 VACCINE INCENTIVE EXTENSION Extend the COVID 19 vaccine incentive that was approved by the Board on 9/1/2021 (C-31-22-118-X-00) to 3/31/2022. Employees must submit proof of being fully vaccinated to Human Resources by close of business 4/6/2022. It is anticipated that the $100 incentive and hours for the day off will appear on the 4/20/2022 paycheck. The hours for the day off must be used by 12/31/2022. This incentive applies to the County’s elected offices, appointed departments, and the Flood Control District of Maricopa County and the Maricopa County Library District (Special Districts). The Board of Supervisors is authorized to jointly adopt policies applying to the Special Districts under the Intergovernmental Agreement, C-06-18-393-6-00, approved on April 11, 2018.
