The benefit offerings will provide Maricopa County employees with six different medical plan options and two pharmacy plan options, allowing them to choose coverage that strikes a balance between health benefit coverage and amount of payroll deductions. Health benefit plan elections will be in force from July 1, 2010 through June 30, 2011. 2. Continue to support program management modifications that advocate the following: Encourage healthy behavior change through incentive-based lower premiums Remove and/or reduce barriers to preventive care Create a culture of health by promoting and incentivizing wellness and disease management Continue the transition to consumerism 3. Continue to support wellness health initiatives through the collection of premium that will be used for such initiatives including, but not limited to, disease management such as diabetic management programs, anti-obesity programs, and tobacco cessation programs; preventive programs, such as on-site mammography, cholesterol and glucose screening, adult immunizations, health risk assessments and various wellness incentives and programs. 5. Continue to offer the following employee only paid benefits: Short-term disability, Flexible Spending accounts, Additional employee, spouse and dependent life insurance, and Legal insurance. 6. Approve the Maricopa County Employee and Employer Health Benefits rates for active employees (full-time and part-time), the pre-Medicare retiree health benefit rates and Medicare-eligible rates for July 1, 2010 through June 30, 2011. See attached rate exhibits. To help encourage employee health behavior changes and improve employee health status, pay period incentives will continue to be implemented for employees who voluntarily participate in the Health Risk Assessment and/or Biometric Screening and for employees and family members who are not tobacco users. The rate attachment illustrates the monthly total, employee cost and employer cost for the health benefit rates for active employees (full-time and part-time), pre-Medicare retirees and Medicare-eligible retirees. For pre-Medicare retiree monthly rates and Medicare-eligible retiree monthly rates, the cost is completely funded by the retiree. There is no employer contribution towards the monthly cost.
