21. APPOINTMENTS TO THE RYAN WHITE PLANNING COUNCIL Approve the appointment of David Garcia- General Public Representative to the Ryan White Planning Council for a three year term, effective on the date of the BOS approval; and approve the appointment of Lisa Fuentes – Alternate for Carla Chee, Service Provider, Ryan White Part B, to the Ryan White Planning Council for the remainder of Carla’s term (May 31, 2015); and approve the appointment of Guillermo Velez – Alternate for Maclovia Morales, Service Provider, Chicanos por la Causa for the remainder of Maclovia’s term (February 28, 2016). Consistent with Article III of the Bylaws of the Phoenix EMA Ryan White Planning Council and after satisfactory completion of the pre-screening processes approved by the Maricopa County Board of Supervisors for existing and new Planning Council members, the County Manager recommends the appointment of David Garcia, General Public Representative, for a three year term, effective on the date of the BOS approval; the appointment of Lisa Fuentes – Alternate for Carla Chee, Service Provider, for the remainder of Carla’s term (May 31, 2015); the appointment of Guillermo Velez – Alternate for Maclovia Morales, Service Provider, for the remainder of Maclovia’s term (February 28, 2016). The Membership Committee of the Ryan White Planning Council has confirmed that the individuals listed above represent membership categories consistent with the Council Bylaws and federally mandated membership requirements.
