35. AMENDMENT TO THE WELLNESS PROGRAMS AND BENEFIT INCENTIVE RATES Approve an Amendment to Wellness Programs and Benefit Incentive rates for FY 2015-16 (approved January 7, 2015) which will allow employees who qualify in Plan Year 2014-15 for wellness premium reduction incentives for biometric screening, health risk assessment and non-tobacco use who elect to participate in Plan Year 2015-2016 County-sponsored medical plans to continue to receive such Plan Year 2014-2015 incentives until they complete the requirements to qualify for Plan Year 2015-2016 incentives or September 27, 2015, whichever comes first. Also, starting with Plan Year 2015-2016, wellness premium reduction incentives will be paid out prospectively once an employee qualifies for them. In previous years, employees had the option of completing requirements to qualify for wellness premium reduction incentives prior to the start of the benefits plan year. For Plan Year 2015-16, eligible employees will be able to qualify for Plan Year 2015-2016 wellness premium reduction incentives by taking biometric screenings, completing health risk assessments, and validating non-tobacco use through the County’s new wellness vendor, Stay Well Health Services, following the start of the plan year on July 1, 2015. This amendment will allow employees who qualified for Plan Year 2014-2015 wellness incentives to continue to receive such incentives for an additional period of time during which they will be able to complete the requirements to qualify for Plan Year 2015-2016 incentives, and therefore will not experience an interruption in their incentives. Total available incentives remain unchanged in Plan Year 2015-2016 at up to $60.00 per month. (C-39-15-015-2-01)

23. AMENDMENT AND TECHNICAL CORRECTION TO NON TOBACCO USE INCENTIVE Approve an Amendment and technical correction to Wellness Programs and Benefit Incentives for Plan Year 2015-16 (approved January 7, 2015 and amended March 11, 2015) by eliminating Exhibits A and replacing it with an amended version of Exhibit B re-designated as Exhibit A. The new Exhibit A makes changes to improve the incentives offered by Maricopa County to encourage and support employees to stop using tobacco products in a manner that is fully compliant with current Federal laws and regulations. Exhibit A also incorporates changes approved in the previously approved amendment (C-39-15-015-2-01), as well as further clarifying changes. Changes are included to enhance and improve the incentive for non-tobacco use based on advice from wellness experts, as well as to insure that the incentive program is in full compliance with current federal laws and regulations. Recent federal regulations require the County to offer employees who use tobacco products a “reasonable alternative” that may be in the form of a requirement to participate in an approved tobacco cessation program. Changes approved by the Board of Supervisors on March 11, 2015 (C-39-15-015-2-01) are consolidated with the original text of C-39-15-015-2-00 Exhibit B, approved January 7, 2015. A statement is added to clarify that the requirements for wellness incentives that are contained in the new Exhibit A supersede all previously-approved program requirements. (C-39-15-015-2-02)
