24. DIABETES MANAGEMENT PROGRAM Approve changing the qualifying period for participants in the Diabetes Management Program (an aspect of the County’s Health and Welfare Benefits Program) from a rolling 12-month period to a period of July 1 through June 30. Although the qualifying period would not be changed until effective July 1, 2017, the Employee Benefits Division is seeking the Board of Supervisors’ approval of this agenda item well in advance of that date to allow sufficient time for this change to be communicated to current program participants. This will allow them time to plan ahead so that they do not have a gap in their medication therapy. (C-39-17-004-2-00)
