36. ADMINISTRATIVE CORRECTION TO IO PHOENIX ONE, LLC CONTRACT AMENDMENT Administrative correction of Amendment No. 1 to Attachment A of Contract with IO Phoenix One, LLC in C-41-14-011-M-01 approved on January 21, 2015, rescinding the same and substituting Amendment No. 1 to Attachment A of the Contract under C-65-10-017-M-02 to allow for extension of the commencement date of the Master Service Agreement and modification of Detail 13 of Attachment A. The County signed a Master Services Agreement with IO Phoenix One, LLC on October 2, 2014 under C-65-10-017-M-01. An amendment was requested under C-41-14-011-M-0 on January 21, 2015. The amendment was requested to extend the commencement date from September 30, 2014 to November 1, 2014. The amendment was attached, but the terms also included deletion in its entirety Detail 13 of Attachment A and replacement with the following: “Licensor shall deliver a dedicated patch panel, located in Licensor’s secure cage with Licensor’s Meet Me Room, directly to the Customer’s Module identifies as SO4.D15, at the Customer’s sole cost and expense.” The modification does not result in any modification of the contract amount. All other terms and conditions of the Agreement not modified in this amendment remain in full force and effect. (C-41-14-011-M-01)

30. ADMINISTRATIVE CHANGE TO RECOGNIZE NEW OWNER AND LICENSOR UNDER LICENSE P-50297 Approve an administrative change to License Agreement No. P-50297 to recognize Iron Mountain Data Centers, LLC, an Arizona foreign limited liability company, as the new property owner and successor Licensor with regards to the License and Master Services Agreement at 615 N. 48th Street, Phoenix, AZ 85008. Pursuant to Section 12.11(b) of license P-50297, approved by the Board of Supervisors via C-41-14-011-M-00, the County received formal notice on February 7, 2018, that Iron Mountain Data Centers, LLC, is the new property owner and successor Licensor of the license agreement via Special Warranty Deed as recorded with the County Recorder on January 11, 2018 via instrument number 2018-0025153. All rent payments shall be sent to IO Phoenix One LLC, PO Box 101188, Pasadena, CA 91189-1187 and Notices under the Agreement shall be sent to Iron Mountain Data Centers, LLC at 615 N. 48th Street, Phoenix, AZ 85008. AMENDMENT TO I/O PHOENIX LEASE P-50297 Approve and execute Amendment No. 2 to Lease No. P-50297 with Iron Mountain Data Centers, LLC for the property located at 615 North 48th Street, Phoenix, Arizona (I/O Phoenix). The purpose of this amendment is to add a dedicated 50m backup data line at this location for the benefit of Correctional Health Services' NaphCare application. This amendment is effective from ten (10) days following full execution of this Amendment to October 31, 2019. As approved on March 21, 2018, Iron Mountain LLC is the successor in interest to I/O Phoenix One, LLC, as Licensor (C-41-14-011-M-03). The Licensor is waiving the installation and monthly service fees associated with this new circuit. (C-41-14-011-M-04)

55. AMENDMENT TO LICENSE P-50297 WITH IRON MOUNTAIN DATA CENTERS, LLC Approve and execute Amendment No. 3 to License No. P-50297 with Iron Mountain Data Centers, LLC for the property located at 615 N. 48th Street, Phoenix, AZ 85008. Iron Mountain is the primary datacenter for Maricopa County systems. The purpose of this amendment is to: a) restate the Office of Enterprise Technology (OET) use and services at this location and update the rates for these services, effective 11/1/2019 for a net savings of $120,235.86 monthly; b) surrender one (1) Dedicated D400 Half Module; and two (2) Dedicated D400 Full Modules to Licensor at this leased location no later than 10/31/19; c) extend the billing term by 61 months to expire concurrently with the other County license in place at this location on November 30, 2024; and d) to provide for 2 five (5) year renewal options. This amendment is effective from November 1, 2019 to November 30, 2024. AMENDMENT TO LICENSE P50297 WITH IRON MOUNTAIN DATA CENTERS, LLC Approve and authorize the Chairman of the Board to execute the Fourth Amendment to License No. P50297 with Iron Mountain Data Centers, LLC, for the property located at 615 N. 48th Street, Phoenix, Arizona, 85008 and direct the Office of Budget and Finance to increase General Fund, Appropriation Unit Enterprise Data Center Systems (EDC0/ITM0) each year as outlined in the attached schedule to align with the additional operating costs under this contract. The cost of extension of the agreement over five (5) years is $9,123,092 plus applicable tax and variable fees. The purpose of this Fourth Amendment is to restate Enterprise Technology’s use and services at this location and update the rates for these services and extend the billing term by sixty (60) months.
