12. AMENDMENT TO ANTHEM SPACE LEASE Approve and execute Amendment No. 5 to Lease No. L-7361 with Anthem Community Council, INS., Sub-Landord, as successor in interest to Del Webb Corporation for the property located at 3701 W. Anthem Way , Anthem , Arizona 85086 . The purpose of this amendment is to extend the term through June 30, 2009, and to implement monthly rental payments of $3,120.38 for months one through seven and of $3,179.25 for months eight through twelve for a fiscal year total of $37,738.91. This amendment is effective from July 1, 2008 to June 30, 2009. The County has occupied office space in Anthem for use by the Sheriff’s Office since November of 2004 and has paid nominal rent during this term of this agreement. An amendment to the sublease is now required to bring the rent to a market level and to extend the term of the sublease through the end of the current fiscal year. This item has been reviewed and approved by the County Facilities Review Committee. (C-50-05-029-M-05)

16. AMENDMENT TO LEASE AGREEMENT WITH DEL WEBB CORPORATION FOR SPACE AT ANTHEM Approve and execute Amendment No. 6 to Lease No. L-7361 with Del Webb for the property located at 3701 W. Anthem Way, Suite 103, Anthem, AZ 85086. The purpose of this amendment is to create a new term and rental rate and to affirm the rental area of the sub leased premises. This amendment is effective from February 1, 2010 to February 28, 2011. This Amendment No. 6 to Sub-Lease L-7361 extends the term of the agreement through February 28, 2011; reduces the annual rental rate to $20.00 per square foot plus applicable taxes beginning February 1, 2010; and denotes Del Webb Corporation as the Sub landlord and as successor in interest to Anthem Community Council. The space is comprised of approximately 1,413 square feet. (C-50-05-029-M-06)
