14. WAIVER TO LEAVE PLAN Approve a waiver to leave plan request for Sheriff's Deputy Sean Crosby who was seriously injured after receiving a blow to the head while participating in a Defensive Tactics Instructor Training course on January 24, 2011. Approval of this waiver allows the deputy to receive full salary and not diminish any accumulated sick leave balances during treatment and recovery. The effective date of this waiver would be April 1, 2011 and would expire one year after approval, unless otherwise authorized. All Workers Compensation checks received by Deputy Crosby for this injury would be returned to the Sheriff's Office. (C-50-11-089-M-00)
